The Prognostic Value of Tumor Multifocality in Clinical Outcomes of Papillary Thyroid Cancer

Abstract: Tumor multifocality has no independent risk prognostic value in clinical outcomes of PTC; its indiscriminate use as an independent risk factor, prompting overtreatments of patients, should be avoided.

“…14,15 Considering that DLI with TPOAb positivity was associated with favorable factors, such as wild-type BRAF and intrathyroidal PTC, this characteristic of multifocality could be confusing to determine whether DLI with TPOAb positivity is a favorable factor for PTC or not. Recently, Wang et al 30 showed that multifocality itself did not have a risk prognostic value in PTC, with a large multicenter study. 28,29 If DLI with TPOAb positivity was just associated with multicentric PTC, it may not be related to bad prognosis.…”

“…28,29 If DLI with TPOAb positivity was just associated with multicentric PTC, it may not be related to bad prognosis. Recently, Wang et al 30 showed that multifocality itself did not have a risk prognostic value in PTC, with a large multicenter study. Although Hashimoto's thyroiditis was not evaluated in the study, the report also supports our suggestion that multifocality found in PTC with DLI may not be associated with bad prognosis.…”

“…Wang et al found that multifocal disease was associated with recurrence (20% vs 14% P < .001) on univariate but not multivariate analysis. The authors then replicated this result in a cohort of 89 680 patients from the SEER database . Other single institutional studies have shown that multifocal disease may be associated with a 0.4%‐4.4% increase in recurrence .…”

“…The authors then replicated this result in a cohort of 89 680 patients from the SEER database. 46 Other single institutional studies have shown that multifocal disease may be associated with a 0.4%-4.4% increase in recurrence. 42,44 It seems therefore that although multifocality is predictive of aggressive disease features, when encountered alone, its impact on outcome is limited.…”

“…7 Our study showed positive and statistical significance between multifocality, age and glandular mass. Some articles [28][29][30] in the literature remain controversial on this point. In spite of the fact that the age of the patients is similar in the studies, some 28,29 point out that there is no relationship or difference in age and glandular mass with unifocal or multifocal lesions, whereas others 30 show a positive relationship between these variables.…”
