The cytokine polymorphisms affecting Th1/Th2 increase the susceptibility to, and severity of, chronic ITP

Abstract: BackgroundT-helper cell type 1 (Th1) polarization in chronic immune thrombocytopenia (cITP) has been reported at the protein and mRNA levels. We evaluated the impact of Th1/Th2 cytokine and cytokine receptor functional polymorphisms on both susceptibility to, and severity of, cITP. We analysed IFN-γ + 874 T/A, IFN-γR -611G/A, IL-4 -590C/T, and IL-4Rα Q576R polymorphisms in 126 cITP patients (male/female: 34/92; median age: 47.7 years) and 202 healthy control donors. Genotyping was determined by PCR and direct … Show more

“…It is widely accepted that naive CD4 + T cells can differentiate into helper T cells such as Th1, Th2, Th9, Th17, and Th22 cells, and the Th1/Th2 imbalance plays an important role in the asthmatic airway (14,28,29). In the OVA exposure experiment, compared with control group, serum IL-4 levels were significantly higher in OVA-induced asthmatic mice, whereas serum IFN-γ levels and the IFN-γ/IL-4 ratio were obviously decreased, indicating that Th1/Th2 balance was disturbed in OVA-induced asthmatic mice (30). It is generally thought that cytokines, IL-12 and IL-4, are crucial for Th1 and Th2 cell differentiation, respectively (31).…”

“…However, T helper (Th) cells compose a broad group of immune cells that includes T helper 1 (Th1) cells, T helper 2 (Th2) cells, T helper 17 (Th17) cells, and T regulatory (Treg) cells. Th1/Th2 and Th17/Treg cell imbalances have been confirmed in ITP patients 3 , 4 . Additionally, the imbalances between these T cells result in an inflammatory state, which contributes to vascular endothelial injury and platelet destruction in ITP patients.…”

“…8 Loss of immune tolerance plays a crucial role in the pathogenesis of ITP, manifested with imbalance of Th1/Th2 and Th17/Tregs, dysfunction of cytotoxic T lymphocytes (CTLs) and NK cells. [43][44][45][46] Monocytes and macrophages are the major cells involved in platelet destruction in the reticuloendothelial system. Surgical splenectomy has been the second-line treatment of ITP for decades with a response rate of 60 to 70%.…”
