Multiregional dissemination of KPC-producing Klebsiella pneumoniae ST258/ST512 genotypes in Poland, 2010–14

Abstract: The study showed remarkable changes in the KPC epidemiology in Poland in 2010-14, which, following the localized regional spread in the early phase, has converted into multiregional dissemination.

“…K. pneumoniae ST258 isolates containing bla KPC in the bacterial chromosome now have been reported by several different groups and may be more common than originally expected (216,217). Sequence types other than ST258 have also been reported, with major representatives including ST11, the predominant type in China and Taiwan, and ST25, ST147, and ST512 in the Americas and Europe (210,213,(217)(218)(219)(220).…”

“…These isolates were divided between two clades, with both bla KPC-2 and bla KPC-3 located on IncFII K2-like plasmids; ST258 clade I was more frequently associated with bla KPC-2, whereas clade II was more frequently associated with bla KPC-3 (210). Large outbreaks of KPC-producing K. pneumoniae in Poland and Colombia have also been attributed to strains belonging to ST258, with bla KPC often found inside the Tn4401 transposon (213,214). In an extensive European genomic study of carbapenem-resistant K. pneumoniae from 244 hospitals in 32 countries, ST258/512 K. pneumoniae, originating in the United States, was identified as the likely progenitor of the large majority of European carbapenemase-producing isolates (215).…”

“…Although there are no previous data on IS disruptions within the O-locus, in general, these elements are common in K. pneumoniae genomes [ 30 ]. The hyperepidemic clone ST258, characterized by the notorious production of KPC-type carbapenemases and extensive drug resistance, has more ISs than an average K. pneumoniae isolate of another ST [ 30 , 31 ]. Several IS types are especially frequent in ST258, such as IS Kpn26 [ 30 , 32 ].…”

“…Several outbreaks of K. pneumoniae infections have been reported in hospital settings from different countries, including China, Israel, Poland, Italy, Colombia, and the United States (Kaye and Dhar, 2016;Ocampo et al, 2016;Baraniak et al, 2017;Krapp et al, 2018;Sotgiu et al, 2018;Liu and Guo, 2019). Reportedly, K. pneumoniae accounts for ∼12% of all hospital-acquired pneumonia in the world (Ashurst and Dawson, 2019).…”
