“…Our case also had features of Purtscher-like retinopathy, including cotton-wool spots, Purtscher fleckens, and compatible findings on OCT and fluorescein angiography. 6,7 In other less-recent reports of Purtscher-like retinopathy that described a hyper-reflectivity in middle and outer retinal layers in their OCT findings (before describing PAMM and AMN as distinct entities,) this hyper-reflectivity similarly resolved spontaneously after several weeks of follow-up. 8,9 This hyper-reflectivity in middle and outer retinal layers cannot be explained by cotton wool spots of Purtscher-like retinopathy, because cotton wool spots are caused by superficial retinal layers ischemia.…”

“…Viral antigens recognized by cytotoxic T cells in infected monocytes elevate pro-inflammatory and vasoactive cytokine titers which lead to capillary leaking, macular edema, and its associated vascular dysfunction. 9 In our patient's SD-OCT there is a sectoral atrophy of the rod and cone layer without edema or hemorrhagic maculopathy, which points to a probable vascular dysfunction of immune etiology.…”
