Background Cardiac amyloidosis, typically from abnormal deposition of AL or ATTR amyloid protein, can result in heart failure requiring transplantation (HTx). The role of mechanical circulatory support (MCS) is not well‐established. The purpose of this study was to present our experience with MCS in patients with cardiac amyloidosis. Methods Consecutive patients with cardiac amyloidosis who received MCS at Cedars‐Sinai Medical Center between 2010 and 2018 were reviewed. Clinical characteristics and outcomes were compared to a control group of MCS patients without amyloid matched 2:1 for age and INTERMACS Profile. Results 11 amyloid patients underwent durable MCS, two with paracorporeal biventricular assist devices and 9 with total artificial hearts. No patients received isolated left ventricular assist device support. By 1 year, 9 (82%) of patients in the MCS‐Amyloid group had been transplanted and 2 (18%) had died. In the MCS‐No Amyloid group, by 1 year, 8 (36%) of patients had been transplanted, 10 (46%) had died, and 4 (18%) were still living with MCS. Conclusions Over a 9‐year period, patients with amyloid cardiomyopathy who required MCS at our institution all received durable biventricular MCS. For carefully selected patients, this approach is feasible with acceptable outcomes as bridge to transplantation.

Immunoglobulin light chain amyloidosis (AL) should be considered in any patient that presents to a cancer care provider with nephrotic range proteinuria, heart failure with preserved ejection fraction, non-diabetic peripheral neuropathy, unexplained hepatomegaly or diarrhea. More importantly, patients being monitored for smoldering multiple myeloma and a monoclonal gammopathy of undetermined significance (MGUS) are at risk for developing AL amyloidosis. MGUS and myeloma patients that have atypical features, including unexplained weight loss; lower extremity edema, early satiety, and dyspnea on exertion should be considered at risk for light chain amyloidosis. Overlooking the diagnosis of light chain amyloidosis leading to therapy delay is common, and it represents an error of diagnostic consideration. Algorithms will be provided on how to evaluate patients with suspected AL amyloid as well as how to manage patients referred from other medical specialties with biopsy-proven amyloid. An organized stepwise approach to the treatment of patients with light chain amyloidosis, including established and investigational therapies, will be reviewed.

W przebiegu amyloidozy łańcuchów lekkich (amyloidozy AL) monoklonalne łańcuchy lekkie immunoglobulin, wytwarzane przez klonalne plazmocyty, odkładają się w tkankach i narządach w postaci nieprawidłowo sfałdowanych, nierozpuszczalnych kompleksów białkowych nazywanych amyloidem. Choroba ma heterogenny obraz kliniczny ze względu na tropizm narządowy prekursorów amyloidu różnorodny u poszczególnych pacjentów. Do najczęściej zajętych narządów należą serce (ok. 80% chorych), nerki, wątroba, a także układ nerwowy. Aby rozpoznać amyloidozę AL, należy przede wszystkim wykryć obecność amyloidu za pomocą barwienia próbki danej tkanki czerwienią Kongo oraz potwierdzić, że składa się on z łańcuchów lekkich immunoglobulin wytwarzanych przez monoklonalny rozrost plazmocytowy. Terapia amyloidozy AL przeważnie jest skierowana na eliminację klonalnych plazmocytów wytwarzających łańcuch lekki immunoglobulin. Kluczowa przed rozpoczęciem leczenia jest klasyfikacja chorych do odpowiedniej grupy ryzyka (niskiego, pośredniego lub wysokiego). W grupie niskiego ryzyka chorzy w młodszym wieku powinni być kierowani na terapię dużymi dawkami melfalanu wspartą przeszczepieniem autologicznych krwiotwórczych komórek macierzystych szpiku kostnego z ewentualnym leczeniem konsolidującym bortezomibem. W najliczniejszej (70% pacjentów) grupie średniego ryzyka standardem jest chemioterapia melfalanem i deksametazonem, a także schematy oparte na bortezomibie. W grupie wysokiego ryzyka zaleca się chemioterapię o zredukowanej intensywności. W leczeniu postaci nawrotowej oraz opornej amyloidozy AL podstawę schematów terapeutycznych stanowią leki immunomodulujące, a także inhibitory proteasomu nowej generacji, bendamustyna oraz przeciwciała monoklonalne.
