Oral health needs of older migrants with culturally and linguistically diverse backgrounds in developed countries: A systematic review

Objectives: To identify and synthesise the available evidence regarding the oral health needs of and the oral health promotion provided for older, cultural and linguistically diverse (CALD) migrants in developed countries. Method: Databases were searched using search terms to identify all relevant Englishlanguage research studies published between 2000 and January 2018. Results: Thirty-nine articles met the review criteria. Thirty-three papers reported on the oral health needs of CALD older migrants and 6 on oral health programs provided for older migrants. Conclusions: Cultural and linguistically diverse older migrants have high oral health needs and poor oral status and face a variety of barriers in accessing oral health care in their host countries. Recommendations to improve their access to oral health programs include building transcultural dental training into the educational requirements of dental students and developing oral health promotion programs that include culturally and linguistically customised information.

Latin American countries have been facing an important social and demographic phenomenon known as South-South migration, which differs from the classic South-North migration pattern due to the lower social and governmental response capacity and similar social and health conditions in both populations (migrants and natives). Oral health is not excluded from this situation and must be analyzed from an integrative proposal that involves demographic, social, and economic factors and those of the migratory process. This article reviews the existing literature in different geographical contexts, identifying conceptual and methodological gaps, and proposing an analysis of migration as a social determinant with an impact on oral health inequalities and inequities. This is the starting point for presenting a conceptual framework that explains the stages and moments faced by migrant groups, identifying the factors that affect the oral health of this population. In conclusion, migration and its relationship with oral health in the Latin American context are in a scientific construction stage and it implies the need to generate research proposals based on the social reality lived by those directly involved and from different methodologies. Similarly, action strategies are required at different levels and with different actors from interdisciplinary approaches, recognizing oral health as a fundamental right linked to general health, for all citizens, regardless of their origin.

Zusammenfassung Hintergrund Personen mit Migrationshintergrund (PmM) unterscheiden sich als Bevölkerungsgruppe hinsichtlich Morbidität, Mortalität und Inanspruchnahme des Gesundheitssystems meist von der autochthonen Bevölkerung, sie nehmen jedoch seltener an Gesundheitsstudien teil. Die Gruppe der PmM ist sehr heterogen, was in Studien bisher kaum berücksichtigt wird. Ziel der Arbeit Es werden soziodemografische Charakteristika der PmM in der NAKO Gesundheitsstudie (Alter, Geschlecht, Zeit seit Migration, Bildung) dargestellt. Zudem wird exemplarisch untersucht, ob der Migrationshintergrund mit der Nutzung des Angebots zur Früherkennung von Darmkrebs (Hämoccult-Test) zusammenhängt. Methoden Daten der ersten 101.816 Teilnehmenden der NAKO wurden deskriptiv und kartografisch ausgewertet. Die Zuweisung des Migrationshintergrunds erfolgte anhand der Definition des Statistischen Bundesamts und basiert auf Staatsangehörigkeit, Geburtsland, Einreisejahr und Geburtsland der Eltern. Ergebnisse Der Anteil der PmM liegt bei 16,0 %. Die Verteilung über die 18 Studienzentren variiert zwischen 6 % (Neubrandenburg) und 33 % (Düsseldorf). Mit 153 Herkunftsländern sind in der NAKO die meisten Länder vertreten. Bei allen Variablen zeigen sich deutliche Unterschiede zwischen den verschiedenen Herkunftsregionen. Am Hämoccult-Test nehmen türkeistämmige Personen (OR = 0,67) und Aussiedler aus der ehemaligen Sowjetunion (OR = 0,60) seltener teil. PmM, die in Deutschland geboren sind, unterscheiden sich diesbezüglich nicht von der autochthonen Bevölkerung (OR = 0,99). Diskussion PmM in der NAKO sind eine sehr heterogene Gruppe. Jedoch lassen sich aufgrund der Stichprobengröße einzelne Untergruppen der PmM hinsichtlich ihrer Herkunftsregion separat untersuchen.

Abstract: Elderly German migrants have higher treatment needs than non-migrants. Likely causes are poorer oral hygiene and lower utilization of dental care services. Specific prevention programs targeting migrants are warranted to improve oral health in this disadvantaged group.
