Mesh fixation with glue versus suture for chronic pain and recurrence in Lichtenstein inguinal hernioplasty

Abstract: Based on the short-term results, glue may reduce postoperative chronic pain and not simultaneously increase the recurrence rate, compared with sutures for mesh fixation in Lichtenstein hernia repair. Glue may therefore be a sensible alternative to suture for mesh fixation in Lichtenstein repair. Larger trials with longer follow-up and high quality are warranted. The difference between synthetic glue and biological glue should also be assessed in the future.

“…[19] Quyn et al found a significantly lower acute and chronic pain with the use of glue. [20] A lower immediate and chronic post-operative pain with the use of glue was also concluded in the study by Sun P et al [21] The usage of glue in mesh fixation has significantly proved its efficacy with regard to decreasing the time needed for mesh fixation in open inguinal hernia repair, as the mean time taken for mesh fixation with glue was 1.73 minutes in our study. Helmy reported a significant decrease in the operative time using Glue mesh fixation for unilateral inguinal hernia repair where 51.2% (22/43) of cases were handled in less than 30 min.…”

“…The authors found that overall postoperative chronic pain in the glue group was reduced by 37% (OR 0.63, 95% CI 0.44 to 0.91; 10 studies, 1418 participants, low-quality evidence) compared with the suture group. 17 Similar low incidence of chronic pain has been reported by the authors such as Quyn AJ et al 18 and Zhang C et al 19 Finally, the analysis of recurrence rates showed that during follow up period only 1 patient in group A reported recurrence whereas there was no case of recurrence in group B. Many authors have including Colvin HS have reported that the recurrence rates after using synthetic glue were comparable to sutures thereby making synthetic glue a better choice for fixation of mesh as compared to sutures.…”
