Tolerance development in food protein-induced allergic proctocolitis: Single centre experience

Abstract: Smaller onset age and onset of symptoms during breastfeeding were found associated with early tolerance development. In the majority of the patients, FPIAP resolves before the age of one year, however in some of the patients this duration may be much longer.

“…Food protein–induced allergic proctocolitis is a non‐IgE‐mediated condition that usually occurs within the first 3 months of life . It represents the benign end of the spectrum of non‐IgE‐mediated allergy, and in about 60% of cases, it occurs in breastfed babies whose mothers are ingesting cow's milk or soya protein .…”

“…Eosinophils may be directly responsible for tissue injury in food protein–induced allergic proctocolitis because they can bind IgA, which are normally present in breastmilk, and undergo degranulation . The majority of infants with food protein–induced allergic proctocolitis become tolerant to offending food allergens by around 14 months of age (±11 months) according to several publications …”

“…As mentioned earlier, AP is characterized by blood-streaked and mucousy normal to moderately loose stools in otherwise healthy, thriving infants. These characteristic symptoms can present within days of birth to 6 months of age, although older presentations have been noted [ 2 , 3 , 16 , 17 ]. Also, there have been cases of the development of AP in children aged 2–14 years [ 18 ].…”
