Vaccines were originally developed to prevent or ameliorate infectious disease. As knowledge of immune function and appreciation of immunodeficiency has developed, researchers have used vaccine responses as a tool to characterize the phenotypes of patients exhibiting various syndromes. Thus it has become possible for a clinician to evaluate individual responses to vaccines to interrogate the immunocompetence of their patients. Although there have been many advances in these areas, we still have much to learn about the quantity and quality of humoral and cellular vaccine responses in healthy and immunodeficient subjects and how that knowledge can then be extrapolated to diagnostic purposes. Adverse effects of vaccines have been recognized for many years, especially the occurrence of infections caused by viable vaccine organisms in immunodeficient hosts. Nevertheless, vaccines are essential for disease prevention in immunodeficient patients, just as they are for healthy subjects. Clinicians must understand the appropriate and safe use of vaccines in patients with immunodeficiency. This review highlights some recent advances and ongoing challenges in application of vaccines for the diagnosis and treatment of immunodeficiencies.

A infeção anogenital pelo vírus do papiloma humano (VPH) é a infeção sexualmente transmissível (IST) mais frequente; devido ao seu potencial oncogénico, está na origem da maioria dos cancros do colo do útero, vulvar, vaginal e anal. Não existe tratamento antiviral específico e o tratamento das neoplasias associadas ao VPH não previne a transmissão. Assim, a prevenção da infeção assume particular relevância e a consulta de Venereologia constitui uma oportunidade privilegiada para aconselhar quem pode beneficiar de medidas preventivas. A mais recente vacina contra o VPH é a nonavalente, proporcionando cobertura para os tipos 6, 11, 16, 18, 31, 33, 45, 52 e 58, que, em conjunto, respondem por 90% dos cancros do colo do útero e condilomas. A vacina é recomendada pela OMS para mulheres e homens até aos 26 anos. Na maioria dos países, a vacina é comparticipada para proporcionar imunização universal gratuita a adolescentes do sexo feminino antes do início da atividade sexual. Para além da população-alvo, a vacina pode beneficiar outros populações-alvo frequentemente tratadas pelos dermatologistas: homens que têm sexo com homens, imunodeprimidos incluindo transplantados e infetados pelo VIH, e candidatos a tratamentos imunossupressores. A decisão de vacinar um indivíduo deve considerar o risco de exposição prévia ao VPH e o benefício potencial da vacinação, que é profilática e não terapêutica. Revemos a evidência publicada sobre a imunogenicidade, segurança e eficácia da vacina contra o VPH em diferentes contextos clínicos e identificamos as recomendações internacionais que podem guiar o aconselhamento individual pelo dermatologista.

Background: When warts appear on the skin, they can cause substantial pain and embarrassment since they are caused by human papillomavirus. Cryotherapy, laser vaporisation and surgical excision are some of the current techniques for treatment of skin cancer There are a number of ways to address these issues, but some of them are more invasive and may result in scarring. A huge number of warts makes local methods impractical for patients. Combining a targeted strategy with an increase of the host immune system has proven successful in treating various lesions. To evaluate the numerous vaccine antigens which was intralesionally injected for tratment of anogenital warts as well as , a comprehensive literature review was conducted. These non-specific intralesional immunotherapies include Candida albicans, mumps, and rubella; Trichophyton; and tuberculin antigens such as pure protein derivative, Mycobacterium w vaccine, and Bacillus Calmette-Guerin. Warts, particularly refractory and anogenital warts, can be treated safely, effectively, and comfortably using intralesional vaccine injection. Bivalent and guadrivalent HPV vaccines are currently available for primary prevention of HPV infection, which includes both types 16 and 18. There has been a dramatic reduction in the incidence of cervical neoplasia as well as genital warts due to HPV vaccines; however, they do not target genotypes that are specific to other skin locations.Objective: To evaluate the potential role of role of HPV vaccines in multiple recalcitrant warts treatment. Conclusion:Several previous studies have demonstrated that recent studies and case reports have shown promising efficacy of HPV vaccines in the treatment of warts.
