Introduction Recent neuroimaging studies have indicated that some brain areas are involved in the sexual behavior process. Pre‐mature ejaculation patients exhibit altered brain activation and abnormal cortical structures. However, there has been no research evaluating the topological organization of structural brain network in anejaculation (AE). Methods We employed diffusion tensor imaging data and graph theory to examine the whole‐brain structural networks among 16 AE patients and 23 healthy controls. In addition, the results of sexual function assessment were also obtained from each patient for symptom assessment. Moreover, the relationships between these clinical features and the altered topological characteristics of AE patients were also explored. Results Decreased clustering coefficient was found in the right amygdala, inferior temporal gyrus, and left pallidum of AE. AE also showed increased path length in the right post‐central gyrus, supramarginal gyrus, putamen, pallidum and left heschl gyrus, temporal pole (superior temporal gyrus); however, these altered brain regions showed no significant differences after false discovery rate correction. Moreover, the path length between subcortical and frontal regions was increased in AE. In addition, negative correlations were found between the altered nodal parameters and the amplitudes of somatosensory evoked potentials of dorsal nerve, sympathetic skin response located in the penis. Conclusion Together, our results suggest that AE was associated with disruptions in the topological organization of structural brain networks. The decreased clustering coefficient in temporal and subcortical regions and the increased path length between subcortical and frontal regions may contribute to AE. These findings provide new insights into the pathophysiological mechanisms of AE.

Hyperactivity of the sympathetic nervous system may play an important role in primary premature ejaculation (PPE), and dapoxetine, a selective serotonin reuptake inhibitor, plays a role in its treatment. We evaluated the success of dapoxetine treatment in PPE patients with a penile sympathetic skin response (PSSR). Of 63 patients who were admitted to our outpatient clinic between March 2017 and December 2017 with a complaint of premature ejaculation and diagnosed with PPE, 56 completed treatment (minimum 12 tablets) with on-demand use 30 mg of dapoxetine and returned for a follow-up appointment. Before and after treatment, the International Index of Erectile Function (IIEF-5) and Arabic index PE (AIPE) scores, intravaginal ejaculation latency time (IELT), testosterone values and PSSR values were compared. The IELT and AIPE values increased significantly in the 56 patients who were re-evaluated after dapoxetine treatment. No significant changes were observed in testosterone levels and IIEF-5. In the PSSR measurement, while the amplitude decreased, the latency was prolonged significantly. PSSR may be an important determinant in the measurement of dapoxetine treatment success for PPE patients, but additional studies are needed.

Characteristically identified as the main component of senile plaques present in patients suffering from Alzheimer's disease, Aβ has been detected in human testis and reproductive fluids, but its effect on spermatozoa has not been addressed. The present study evaluated whether the most toxic and aggregant amyloid precursor protein (APP)-proteolytic product, amyloid-β (Aβ), was capable of affecting sperm functionality. Normozoospermic samples were either exposed to different Aβ doses or to the untreated and scrambled controls for a maximum of 48 h at 37 °C and 5%CO, and motility, viability and mitochondrial status were evaluated. Additionally, tyrosine phosphorylation was analyzed by immunocytochemistry and acrosomal integrity through PSA-FITC. A shorter treatment period was used to monitor prompt Ca responses. Aβ peptide decreased motility before inducing mitochondrial impairment (p < 0.05; n = 6). Both outcomes became more pronounced with time, reaching their maximal decrease at 48 h, where even 1 μM produced undesirable effects (p < 0.05; n = 6). Aβ peptide also decreased cell survival (p < 0.05; n = 6). Furthermore, although no effects on tyrosine phosphorylation were observed (p > 0.05; n = 6), reduced acrosomal integrity was detected (p < 0.05; n = 7), which was not correlated with viability loss (p > 0.05). In parallel, all Aβ concentrations elicited a [Ca] rise but a significant difference was only observed at 20 μM (p < 0.05; n = 7) and a tendency was obtained with 10 μM (p = 0.053; n = 7). In conclusion, Aβ peptide oligomers impair sperm function in vitro, although further studies are required to determine the clinical relevance of these findings.

Abstract: These results showed that patients with lifelong PE had higher APP and Aβ42 levels in seminal plasma. Abnormal PSSR was related to a higher Aβ42 level. Drugs that decrease Aβ could be treatment of PE.
