ABSTRACTPrism adaptation (PA) alters spatial cognition according to the direction of visual displacement by temporarily modifying sensorimotor mapping. Right-shifting prisms (right PA) improve neglect of left space in patients, possibly by decreasing activity in the left hemisphere and increasing it in the right. Left PA shifts attention to the right in healthy individuals by an opposite mechanism. However, functional imaging studies of PA are inconsistent, perhaps because of differing activation tasks. We measured resting-state functional connectivity (RSFC) in healthy individuals before and after PA. Right, vs. left, PA decreased RSFC in the navigation network defined by the right posterior parietal cortices (PPCs), hippocampus, and cerebellum. Right PA, relative to baseline, increased RSFC between regions within both PPCs and between the PPCs and the right middle frontal gyrus, whereas left PA decreased RSFC between these regions. These results show that right PA modulates connectivity within the right-hemisphere navigation network and shifts attention leftward by increasing connectivity in the right frontoparietal network and left PA produces essentially opposite effects, consistent with the interhemispheric competition model. These finding explain the action of PA on intact cognition and will help optimize interventions in neglect patients.

Language-Specific Dysgraphia in Persian Patients with Right Brain Stroke: Influence of Visuospatial Neglect and Rehabilitation

Background:Writing is a human communication tool. No disability behavioral assessment of dysgraphia due to stroke-induced neglect has been conducted in Iran. Objectives: We investigated rehabilitation of dysgraphia in patients with stroke-related neglect in Persian language-specific constructive errors of writing. Methods: All patients were suffering from stroke-induced visual neglect. Five patients were evaluated for dysgraphia in a spontaneous writing task before and after rehabilitation without writing practice for 10 sessions with prism adaption (PA). Dysgraphia was classified into visuospatial omission error, visuospatial destruction error, visuospatial size error, visuospatial tilting error, poor handwriting style, visuospatial distance error, and visuospatial perseveration (addition) error. Neglect was evaluated using the star cancellation test (SCT), and line bisection test (LBT) and activities of daily living (ADL) was assessed by the Barthel index (BI) and the Catherine Bergego scale (CBS). Results: All patients showed significant improvement in dysgraphia (measured using spontaneous writing test), ADL (measured using CBS and BI), and neglect (measured using LBT and SCT) (P < 0.05). Visuospatial destruction errors were the most frequent and visuospatial omission errors were the least frequent errors. Conclusions: PA is a potential effective strategy in stroke rehabilitation of dysgraphia, visuospatial neglect and ADL. Unilateral spatial neglect and rehabilitation improves dysgraphia in Persian language speaking patients with right brain stroke.

The review highlights the fundamental principles of early rehabilitation in ischemic stroke, the benefits and risks of early and very early patient mobilization. It presents data on the efficiency of CIMT-kinesiotherapy and mirror therapy in restoring upper extremity function, as well as procedures for nonpharmacological correction of spatial neglect syndrome. The effect on the rehabilitation of concomitant Alzheimer's disease is analyzed. The areas of pharmacological potentiation of poststroke rehabilitation, the efficiency of cerebrolysin in particular, are considered.
