Magnetic resonance texture parameters are associated with ablation efficiency in MR-guided high-intensity focussed ultrasound treatment of uterine fibroids

Abstract: Uterine fibroid texture parameters provide complementary information to T2Wr, and are associated with MRgFUS efficiency. Key points Mean sum entropy is negatively correlated with MRgFUS efficiency (ρ = -0.307, p = .021). Fibroids/muscular T2-weighted ratio and entropy are associated with MRgFUS efficiency. Texture parameters are better predictors of MRgFUS efficiency than Funaki type. Fibroid MR texture analysis can improve patient selection for MRgFUS.

“…DWI can provide information about tissue structure, cellularity, and microcirculation [18], a previous study [31] indicated that DWI of uterine fibroids could provide information about both diffusion and perfusion effects, which are crucial determinants of the HIFU treatment outcome. Previous studies [11,[13][14][15] with MRI parameters have revealed correlations with treatment efficiency parameters but they are difficult to apply in clinical use because there are no clear guidelines for the prediction of the treatment outcome by inspection of the treatment efficiency values. Classification with the NPVr could be a more informative as well as a more straightforward way to apply quantitative information into clinical use because NPVr correlates with clinical success [5,6].…”

“…Both heating and ablation efficiencies of MRgHIFU were investigated for each fibroid. Heating efficiency was defined as the total volume of 240 CEM43 contours divided by the total volume of treatment cells (%), and ablation efficiency was defined as the nonperfused volume divided by the total volume of treatment cells (%), similarly as in previous studies [13,15,22]. Available treatment cell sizes were 4, 8, 12, 14 and 16 mm, corresponding to estimated ablation volumes of 0.08, 0.67, 2.26, 3.59 and 5.36 mL, respectively.…”

“…The main problems related to MRgFUS/MR-HIFU are of logistic nature, e.g., preparation of the patient and device, qualification and a possible problem of cooperation between the radiologist and gynecologist. If both of them are sufficiently engaged and experienced in this cooperation, the total UF treatment risk is then calculated as very low and the efficacy as higher [64,65]. Moreover, advanced research is conducted in order to increase the effectiveness of MRgFUS/ MR-HIFU therapy, e.g., with the use of oxytocin [66].…”

“…Polygonal ROIs were drawn manually on EOMs and OF, respectively. After ROIs were placed, following quantitative texture parameters of EOMs and OF, including the entropy, uniformity, skewness and kurtosis, were obtained automatically by using AK software (Artificial Intelligence Kit, GE Healthcare) ( 16 ).
