Gastric Perforation due to Giant Trichobezoar in a 13-Year-Old Child

Abstract: Trichotillomania and trichotillophagia can cause the formation of enormous intragastric hairballs. We report the case of a 13-year-old girl who was brought to the emergency service for evaluation of an acute abdomen. Abdominal CT scanner showed a giant gastric trichobezoar which had to be removed by susombilical laparotomy and transverse gastrotomy. This case illustrates the fairly uncommon perforation risk of these gastric bezoars.

“…Primary or recurrent cases of trichobezoars may lead to complications such as intussusception[ 4 , 5 ], pancreatitis[ 6 ] and bile duct dilatation[ 4 ]. Significant other complications such as gastric perforation[ 7 , 8 ], peritonitis[ 9 ], and even death[ 10 ] have also been reported. Despite the potential for significant complications and mortality, there is still a lack of any specific and comprehensive guidelines on appropriate postoperative follow-up for patients with Rapunzel syndrome to reduce the risk of recurrence[ 11 ].…”
