Indirect Calorimetry in Mechanically Ventilated Patients: A Prospective, Randomized, Clinical Validation of 2 Devices Against a Gold Standard

Abstract: The QUARK RMR ICU Indirect Calorimeter compared better with the gold standard for values of [Formula: see text]O and REE than did the CCM Express Indirect Calorimeter in mechanically ventilated patients who were circulatory and respiratory stable. Both indirect calorimeters had low precision.

“…Finally, another possible limitation should be acknowledged. Evidence exists on the inconsistency of results obtained from the different commercialized indirect calorimeters . In accordance with these studies, CCM Express overestimates REE in comparison with the Quark RMR ICU indirect calorimeter (Cosmed, Rome, Italy), Deltatrac II (Datex, Helsinki, Finland), and modified Tissot bell‐spirometer .…”

“…We could not detect impact of these two monitoring principles in our analysis. Indirect calorimetry has the advantage of being continuous and has proven feasibility during anaesthesia in semi‐closed circle absorber systems . The pulmonary artery catheter has no clear benefit in non‐cardiac surgery and its use has declined .…”

“…Therefore, our study may be underpowered in relation to the clinical effects of DJBS. Ultimately, as the use of indirect calorimetry, at least in the intensive care unit, has been shown recently to be less accurate as compared to gold standards, we may have overlooked a potential effect of DJBS on energy expenditure.…”
