A comparison of surgical outcomes and complications between hemostatic devices for thyroid surgery: a network meta-analysis

Abstract: This meta-analysis compared harmonic scalpel and LigaSure systems with the conventional clamp-and-tie technique in thyroidectomy. Medline, Cochrane, EMBASE, and Google Scholar databases were searched until December 30, 2015. Randomized controlled studies (RCTs) or two-arm prospective studies were included. The primary outcome was operation time. The data were evaluated both by pair-wise meta-analyses and network meta-analysis within a Bayesian framework using Markov chain Monte Carlo methods. Compared with the… Show more

“…This systematic review updates the data available in the literature with published clinical trials up to the mid-2017. The clinical data found in this study are in line with the latest published reviews on the subject [51][52][53][54] and show that there is apparently no doubt about the actual efficacy and safety of the harmonic scalpel, questioning only the cost-effectiveness of the implementation of this technology in thyroid surgery.…”

“…Our observations were similar to those of previously published reports. 19 - 21 This result confirms the findings that morbidity secondary to thyroid surgery largely depends on the surgical skill, and not so much on the kind of intraoperative procedure used to achieve hemostasis.…”

“…In a recent meta-analysis published in 2017, Luo et al compared new hemostatic scalpel with the conventional technique used during thyroidectomy. They found that the new hemostatic devices had less operation time, less risk of definitive recurrent laryngeal nerve paralysis, low intraoperative blood loss and postoperative bleeding than the conventional hemostasis (19).…”
