Evaluation of the measurement properties of symptom measurement instruments for atopic eczema: a systematic review

Abstract: ISS (paediatric version), POEM, PO-SCORAD, SA-EASI and adapted SA-EASI are currently the most appropriate instruments and therefore have the potential to be recommended as core symptom instrument in future clinical trials. These findings will be utilized for the development of a core outcome set for AE.

“…To determine the overall level of evidence for a measurement property of an instrument, data were synthesized by combining the quality of the measurement property results, the methodological study qualities and the consistency of the findings [18,19] ( Table 1). “…Accurate symptom monitoring by patients or their caregivers is a basic component of effective management of allergic diseases, but previous monitoring tools for AD were not sufficiently sensitive . Therefore, we explored a new symptom monitoring tool, the ADSS, which would permit daily evaluation of AD status and help to determine individual‐specific aggravating factors.…”

“…To date, there have been a few self‐monitoring tools for AD, such as the Patient‐Oriented Eczema Measure (POEM), the Patient‐Oriented SCORing Atopic Dermatitis (PO‐SCORAD) Score, and the Self‐Administered Eczema Area and Severity Index (SA‐EASI) . Although the international Harmonising Outcome Measures for Eczema (HOME) initiative recommended the POEM as the best core instrument to measure AD symptoms, a recent systematic review pointed out that the evidence for internal consistency was limited, and the test‐retest reliability was unknown . Moreover, the sensitivity and specificity of POEM were only 62.1% and 66.1%, respectively, at the receiver‐operating characteristic (ROC) cutoff points .…”

“…Parents or guardians were instructed to record AD symptoms on a daily basis. Various instruments have been used to assess AD symptoms, but the quality of measurement varies . In the present study, the symptom diary was designed for the parents to easily measure AD symptom severity by describing the extent of itching and sleep disturbance and the degree of erythema, dryness, oozing, and edema on a scale of 0‐4.…”
