Aim: The body’s antioxidant system processes rely on thiol-disulfide equilibrium to function correctly. The purpose of this study was to look into dynamic thiol/disulfide homeostasis (TDH) as an endogenous antioxidant indicator in febrile neutropenic (FN) children with acute lymphoblastic leukemia (ALL). Materials and methods: The study included 66 acute lymphoblastic leukemia patients, including 33 with FN and 33 controls. Both groups were compared for TDH and ischemia-modified albumin (IMA). TDH parameters were assessed using a unique spectrophotometric approach disclosed earlier by Neşelioğlu and Erel. Results: Total thiol, native thiol, disulfide, and native thiol/total thiol levels were significantly lower in patients with FN than in the controls (p< 0.05). Disulfide/total thiol, disulfide/native thiol, and IMA levels were significantly higher in the FN group than in controls (p<0.05). Conclusions: TDH could be a simple tool that could be used in routine screening as an indicator of thiol-specific oxidative stress, particularly in pediatric febrile neutropenia. Aims and Objectives: To find out if ischemia modified albumin can be a novel marker in ischemic heart disease. Materials and methods: Retrospective chart-review of chest pain patients who were evaluated in the emergency departments in 2006 and 2007. Results: Total of 400 patients presented with chest pain and subsequently were evaluated in the Emergency department at two major emergency departments associated with the teaching hospital associated Mercy Catholic Medical Center; few patients were excluded due to unavailability of complete data and follow-up visit in next six months. Thus, finally 351 patients met criteria for final analysis. Primary endpoints included the cardiac events and secondary outcomes were course in the hospital, days of hospitalization, interventions done in the hospital, disposition from the hospital, and readmission within next six months. Sensitivity and specificity of ischemia modified albumin (IMA) test was 80% and 50% respectively and positive predictive value and negative predictive values were 17% and 88% respectively. There were no further differences in cardiac events or interventions in higher ischemia modified albumin (IMA) group as compared to troponin (cTnT) (p=0.5). People with higher ischemia modified albumin (IMA) showed longer hospitalization days (p=0.025), needing nursing home or skilled nursing facility on discharge due to high discharge needs (p=0.0001) and had more readmissions as compared to troponin (p=0.02). However, Higher ischemia modified albumin (IMA) did not predict more cardiovascular events during hospital stay, rather troponin (cTnT) test predicted arrhythmia more than ischemia modified albumin (IMA) test (p=0.0001). Conclusion: Ischemia-modified albumin may be utilized as a novel marker of ischemia to rule out acute coronary syndrome along with troponin and electrocardiogram in the emergency departments. This test may also have short and long-term prognostic significances and may even be used as a surrogate marker in stroke, sepsis and thrombo-embolic diseases once these people need admission to the hospital.

Purpose: This study aimed to review the potential role of ischemia-modified albumin as a biomarker for diagnostic modalities in the ophthalmology field. Methods: Articles were reviewed without a specific date. A manual search was also performed by reviewing reference lists of meta-analyses and systematic reviews. All articles were reviewed, and a total of 18 articles were selected by the authors. Results: Oxidative stress increases structural and functional damage to proteins in many ocular diseases. The human serum albumin is a major circulating protein with antioxidative and anti-inflammatory properties. Oxidative stress has been shown to be an important part of etiology and pathogenesis in ocular diseases related to ischemia. Biomarkers that are specific to oxidative stress and ischemia-related ocular pathogenesis are needed to provide an extensive understanding regarding diagnosis, monitoring progression, and new potential target treatment. Ischemia-modified albumin (IMA) as a new promising biomarker might be useful in the early detection and treatment of ocular diseases with ischemic pathogenesis. Conclusion: IMA plays an important role in the progression of ophthalmology diseases, such as diabetic retinopathy, hypertensive retinopathy, cataract progression, seasonal allergies, and glaucoma. Further studies are needed to elaborate these results as a consideration in new testing modalities in clinical practice as well as a new target therapy research.
