Rare onset symptoms in multiple sclerosis

Abstract: This retrospective study indicates that there is a wide range of rare initial manifestations of MS due to diverse involvement of central nervous system. Rare, unusual symptoms complicate early diagnosis. Given the fact that disease modifying treatment should be initiated early, it is crucial to diagnose MS as early as possible. Therefore, physicians should not disregard MS when a patient presents with any of these rare manifestations.

“…Finally, we found a correlation between an older age of initial evaluation by a DD specialist and the latency between the onset of DD and MD, suggesting that a shorter latency between DD and MD was associated with an earlier assessment by a DD specialist. We suspect that the presence of MD could cause patients to seek neurological evaluation earlier, while atypical onset of MS with a MD has been associated with a delay in the final diagnosis in other studies 17,18…”

“…We suspect that the presence of MD could cause patients to seek neurological evaluation earlier, while atypical onset of MS with a MD has been associated with a delay in the final diagnosis in other studies. 17,18 Our study has the limitations inherent to retrospective designs with occasional incomplete data. In addition, the response to medications was not assessed using validated objective MDs scales pretreatment and post-treatment, patients were not randomized, and patient and clinicians were not blinded to the treatment.…”

“…The wide variety of visual complaints in multiple sclerosis can lead to clinical confusion [14,15]. Recent literature highlights the unusual semiology of this autoimmune demyelinating disorder (pain syndromes, movement disorders, rare cranial nerve involvement, cognitive or psychiatric symptoms [16,17]. In this case, both a late onset and an atypical clinical pattern were misleading.…”

“…Rare cases of MS have been reported in the literature. 24 Among the cases we followed, neuropathic pain occurred as the first symptom in a total of 3 cases. In female patients, L-hermitte and neuralgia in the trigeminal region were described, while in male patients, neuropathic pain in the left hemibody was reported.…”
