Endoscopic ultrasound in the diagnosis and management of carcinoma pancreas

Abstract: Endoscopic ultrasound (EUS) has become an important component in the diagnosis and treatment of carcinoma pancreas. With the advent of advanced imaging techniques and tissue acquisition methods the role of EUS is becoming increasingly important. Small pancreatic tumors can be reliably diagnosed with EUS. EUS guided fine needle aspiration establishes diagnosis in some cases. EUS plays an important role in staging of carcinoma pancreas and in some important therapeutic methods that include celiac plexus neurolys… Show more

“…EUS seems to be the most sensitive method for diagnosing (excluding) ductal adenocarcinoma of the pancreas [ 65 ]. The high resolution of the method can visualize pancreatic lesions as small as 5 to 15 mm [ 66 ].…”

“…Die Sonografie und die Endosonografie (EUS) ergänzen die konventionellen Verfahren. Mittels EUS können Läsionen bis zu 20 mm mit einer Sensitivität von 93 -100 % dargestellt werden [7,8]. Ein weiterer Vorteil der EUS im Vergleich zum CT ist, dass Lymphknotenmetastasen sowie Gefäßinfiltration mit einer höheren Sensitivität nachgewiesen werden können.…”

“…7 Given the lengthier perfusion duration, CEH-EUS performs real-time vessel imaging of pancreatic lesions without Doppler-related artifacts. 10,11 Napoleon et al 12 performed a pilot study in 35 patients with SPL using SonoVue ® with a novel Olympus echoendoscope able to detect extended harmonics (XGF-UCT 180; Olympus America, Melville, NY, USA). 12,13 Of the 18 fine-needle aspiration (FNA)-confirmed ductal adenocarcinomas, 16 resulted in a hypointense signal on CEH-EUS.…”

“…7,8 Contrast-enhanced Doppler EUS, however, remains limited in use secondary to blooming artifacts and lower sensitivity in vessels with low blood flow. 7,9,10 To overcome these limitations, a new generation of intravenous contrast agents, such as SonoVue ® and Echogen ® , which use gas-containing micro bubbles encapsulated by albumin or a phospholipid that oscillate in response to sound pressure, have been developed. 7 Given the lengthier perfusion duration, CEH-EUS performs real-time vessel imaging of pancreatic lesions without Doppler-related artifacts.…”
