Hearing loss in survivors of childhood head and neck rhabdomyosarcoma: a long‐term follow‐up study

Abstract: One in five survivors of HNRMS developed clinically relevant hearing loss. AMORE-based treatment resulted in less hearing loss compared to EBRT-based treatment. As hearing loss was highly prevalent and also occurred in survivors with orbital primaries, we recommend systematic audiological follow-up in all HNRMS survivors.

“…Treatment details for this cohort have been described previously; in general, all patients received multidrug chemotherapy and decisions on local therapy were made after two or three courses of chemotherapy. If local therapy was indicated, the patients from the United Kingdom (UK) received external beam radiotherapy and the EKZ‐AMC patients received AMORE (Ablative surgery, MOuld technique after loading brachytherapy, and surgical REconstruction) treatment if feasible and otherwise external beam radiotherapy . AMORE treatment was considered feasible if a macroscopic radical resection and adequate mould placement seemed possible.…”

“…Radiation involving the ear or auditory nerve is often associated with ototoxicity . Nevertheless, radiation may be required to attain curative treatment for many central nervous system tumors, as well as for malignancies involving structures in the head, such as rhabdomyosarcoma and nasopharyngeal carcinoma . The effects of radiation to the ear appear to be dose‐related, with sensorineural hearing loss generally occurring at doses >30 grays .…”

“…Patients with head and neck RMS treated with EBRT were found to have higher incidences of hearing loss compared to survivors who had received AMORE based therapy. [12] While our patient's treatment did not follow the AMORE protocol, her treatment did consist of the use of brachytherapy rather than EBRT. EBRT in head and neck RMS patients is also associated with an increased risk to develop any grade 3 event or more than 5 adverse events compared with survivors who received brachytherapy based treatments.…”

“…In the head and neck region ERBT can result in hearing loss, facial hypoplasia, developmental delay, xerostomia, dental problems, problems with the pituitary/hypothalamic hormones and second malignancies. [12,13] Brachytherapy is comparatively less damaging because of the localized mechanism of action, which should spare the surrounding tissue.…”
