Introduction: Congenital clubfoot (CC) is an orthopedic deformity characterized by congenital dysplasia of musculoskeletal structures such as muscles, tendons, ligaments, osteoarticular and neurovascular structures, of multifactorial and idiopathic origin. Objective: Offer early intervention for the treatment of congenital clubfoot in newborns, using ethyl vinyl acetate (EVA) to produce braces in the neonatal period. Method: Intervention was performed in newborns of both sexes, with gestational age >37 weeks, diagnosed with flexible congenital clubfoot. The braces were manufactured with 4 mm-thick EVA, Velcro and hot glue, using flat irons and scissors.Results: Fifteen newborns were included in the study, with bilateral CC and 32 braces. An average of 20 cm 2 of EVA was used for each brace and none of the newborns developed pressure sores. Braces were worn initially for 18 hours and no family member expressed any complaint or difficulty in handling the device at home, reporting complete satisfaction. After 60 days of treatment, all the infants in the follow-up achieved a score of 0 on the Pirani scale, but were monitored for a further 30 days because they did not exhibit symmetrical feet. Multivariate analysis revealed differences between assessments (p = 0.001), and in pair comparison, only a score of 30 after 60 days was similar (p = 0.45). Conclusion: Rapid intervention, performed by a qualified professional using EVA braces, proved to be effective for conservative treatment of CC in newborns. Objetivo: Oferecer intervenção precoce no tratamento de pé torto congênito de recém-nascidos e apresentar o uso de Etil Vinil Acetato (EVA) como um material para produção de órteses ainda no período neonatal. Método: Foi realizada intervenção em recém-nascidos de idade gestacional >37 semanas, de ambos os sexos, diagnosticados com pé torto congênito flexível. As órteses foram desenvolvidas com EVA de espessura 4 mm, velcro e cola quente, utilizando-se prancha de cabelo e tesoura para confecção. Resultados: Foram incluídos 15 neonatos no estudo, com PTC bilateral. Para cada confecção de órtese foi usado em média 20 cm 2 de EVA e nenhum recém-nascido evoluiu com úlcera de pressão. As órteses foram mantidas por 18 horas inicialmente e nenhuma família apresentou qualquer queixa ou dificuldade em manipular o material em casa, relatando integral satisfação. Background Treatment of idiopathic congenital talipes equinovarus (CTEV) is challenging for pediatric orthopedic surgeons. The Ponseti method is an effective protocol for treatment due to its technique of manipulation, casting, and limited surgery. Plaster casting is an essential component of the Ponseti method. In this report, we describe a new brace that was developed for use in the treatment of clubfoot in newborns instead of a plaster cast. Methods This retrospective study was performed in two orthopedic medical centers. Between January 2011 and October 2013, 89 newborns with CTEV (131 ft) underwent corrective treatment using fixation braces in the experiment group (E-group) in our hospital, and 107 newborns with CTEV (141 ft) underwent plaster casting in the control group (C-group) in another medical center. All patients were treated according to the Ponseti method after the application of the inclusion and exclusion criteria. Plaster casts were applied to patients in the C-group. The patients in the E-group received the custom-made polyaxial fixation braces instead of plaster casts. Prospective follow-up was performed for a mean duration of 36 months. The efficacy of the treatment was assessed using Pirani’s scoring system. Chi-squared and independent t tests were used for statistical analyses. Results In the E-group, 85 patients (125 ft) achieved good appearance within 3 months of treatment initiation (average, 1.7 months). Four patients (6 ft) required percutaneous Achilles tenotomy. Seven patients developed sores during treatment because of improper brace application, but all sores healed without scarring with timely treatment. In the C-group, 96 patients (123 ft) achieved good appearance within 3 months of treatment initiation (average, 1.6 months). Eleven patients (18 ft) required percutaneous Achilles tenotomy. Twenty-one feet developed sores during treatment because of plaster cast pressure on the dorsum of the feet. Sixteen sores healed without scarring with timely treatment, and 5 ft had obvious scars. The overall mean Pirani scores 1 year after treatment were 0.26 ± 0.06 in the E-group and 0.25 ± 0.03 in the C-group, and the Pirani scores 3 years after treatment were 0.23 ± 0.05 in the E-group and 0.22 ± 0.03 in the C-group. There were significant differences in the percutaneous Achilles tenotomy and skin sores but no significant difference in the Pirani scores between these two groups. Conclusions Our results showed that the new polyaxial fixation brace used in this study was an effective tool for the corrective treatment of CTEV in newborns. We propose the use of this brace as an alternative treatment for newborns.

INTRODUCTION: Early intervention is essential for proper foot growth in postural congenital clubfoot (PCC), but little is known about its contribution to this deformity when subjects are evaluated through telemonitoring. OBJECTIVE: This study aimed to monitor the foot’s flexibility of newborns diagnosed with PCC by telemonitoring them during the first months of life. METHODS: A longitudinal descriptive study was carried out with a full-term newborns group diagnosed with PCC in at least one limb, presenting a grade ≥ 0,5 on the Pirani score. Newborns with other malformations were excluded. They were assessed twice: before and 30 days after hospital discharge, and the foot flexibility classification by the Pirani score was provided. The telemonitoring occurred weekly between the assessments, and the parents were encouraged to mobilize their feet and maintain foot position using orthosis or taping. RESULTS: Thirteen newborns (eighteen feet) presenting PCC were included in this study; seven neonates discontinued the study due to absences from pre-scheduled evaluations, and six were telemonitored for 30 days. They were born at 39 weeks (± 1.18) and 3346.54 g (± 306.51). The majority of the newborns were female (69%), one was born vaginally, and eight (61%) had a family history of PCC. Pirani's score ranged from 1 to 3 in the initial assessment. After one month of telemonitoring, three feet progressed to 0, and four feet scored between 0.5 and 1. CONCLUSION: This study shows an important improvement in the foot’s flexibility of newborns diagnosed with PCC evaluated through telemonitoring. Telemonitoring may be an additional resource for assisting newborns with PCC.
