Melioidosis is caused by Gram-negative bacterium Burkholderia pseudomallei. Clinical presentation can vary from pneumonia, sepsis and multi-focal abscess formation. The aim of this study was to systemically review the cardiac manifestations of melioidosis in the literature and describe their epidemiology, microbiological diagnosis and outcomes. A systematic review of the peer-reviewed literature was carried out in PubMed and Google Scholar for human melioidosis cases with cardiac involvement. Quantitative data for cases of melioidosis were obtained, including age, sex, microbiological diagnosis, treatment, and outcome. 980 articles were screened, of which 31 articles were eligible. The most common cardiac site of infection was pericarditis, followed by endocarditis and myocarditis. Over 95% of cardiac involvement occurred in males, and mortality was the lowest in pericarditis and highest in myocarditis. Valvular vegetations were all small, left-sided, and did not require surgery. Antibiotic treatment included a bactericidal induction therapy with ceftazidime or a carbapenem ± TMP-SMX, followed by eradication therapy with TMP–SMX in most patients as previously established. In conclusion, melioidosis varies in clinical presentation and is also known as a great imitator. Although cardiac involvement is rare, this is the first systematic review to summarise all cases reported in the literature to date.

The occupation of commercial fishing is recognized as a comparatively stressful and hazardous job characterized by a myriad of risks to health, including high rates of job‐related injury, morbidity, and mortality. Nonetheless, systematic study of fisher health is limited. This paper examines a particularly understudied aspect of illness among fishers: the physical and social contexts, pathways, and potential for adverse disease interactions. The focus of the paper is on a biosocial disease process called syndemics, which increases the health burdens of this population. The paper is presented as an initial testing of the hypothesis that a “lens” of syndemics improves occupational health research and advocacy for commercial fishermen who face a substantial burden of biosocial challenges.

Certain infectious diseases have a higher frequency and severity in patients with diabetes mellitus (DM), which substantially increases morbidity and mortality rates. DM acts as a predisposing factor for bacteremia seen in both the nosocomial and community settings. The greater incidence of infections in diabetics is caused by the accompanying hyperglycemia, which generate an immune dysfunction characterized by disorders of the neutrophilic granulocytes, diminished antioxidant activity and humoral immunity. Other complications of DM such as micro or macroangiopathy, neuropathy, urinary and gastrointestinal motility disorders, among others, contribute to the pathogenesis of infectious processes in these patients. It is thought that cross-linking of advanced glycation end products is associated with inactivation of immune components, contributing to a higher vulnerability to infections. An association between hyperglycemia and infection in critically ill patients has been proposed; however, evidence remains limited. Several risk factors infl uence negatively in the prognosis of severe sepsis, such as advanced age, immunosuppression and chronic alcoholism. There still is contradictory data concerning the impact that DM could exert on the prognosis of septic patients.
