Background As epidemiological data on child mental health in low- and middle-income countries are limited, a large-scale survey was undertaken to estimate the prevalence and amount of child emotional and behavioral problems (EBP) in Nepal as reported by the parents. Methods 3820 schoolchildren aged 6–18 years were selected from 16 districts of the three geographical regions of Nepal, including rural, semi-urban and urban areas. We used the Nepali version of the Child Behavior Checklist (CBCL)/6-18 years as screening instrument. Comparisons of child problems between genders and between the seven largest castes and ethnic groups were carried out by analysis of variance. Prevalence was computed based on American norms. Results Adjusted prevalence of Total Problems was 18.3% (boys: 19.1%; girls:17.6%). The prevalence of internalizing problems was higher than externalizing problems. The mean scores of Total, Externalizing, and Internalizing problems were 29.7 (SD 25.6), 7.7 (SD 8.0), and 9.1 (SD 8.1), respectively. The Khas Kaami (Dalit) group scored the highest, and the indigenous Tharu group scored the lowest on all scales. In the Mountains and Middle Hills regions, problem scores were higher in the rural areas, whereas in the Tarai region, they were higher in the urban areas. Conclusion The prevalence and magnitude of emotional and behavioral problems in Nepali children were found to be high compared to findings in meta-analyses worldwide. Problem scores varied according to gender, castes /ethnic groups, and living areas. Our findings highlight the need for a stronger focus on child mental problems in a low-and middle-income country like Nepal.

Background.Mental health issues, often manifested as behavioural difficulties, in children are estimated to be high in low and middle-income countries (LMIC) settings. There is a paucity of definitive data due to a lack of well-validated tools to use across settings. This review aims to provide evidence on what tools are used and which have been adapted and validated in LMIC settings.Methods.We performed a systematic review to identify tools used to assess or screen externalising behaviour problems in children and adolescents in LMIC and assess their cultural adaptations. We searched for studies measuring externalising behaviour in children from 0 to 19 years published up to September 2018. Articles were assessed to identify tools used and analysed using the Ecological Validity Framework.Results.We identified 82 articles from over 50 LMICs who had studied externalising behaviour in children. Twenty-seven tools were identified, with a predominance of studies using tools from the USA and Europe. Most studies did not describe an adaptation and evaluation process, with only one study following recommended criteria. New tools were identified which both screen and assess externalising behaviour which have not yet been utilised across settings.Conclusions.Although tools from the USA and Europe are often utilised to screen and assess for externalising behaviour problems in children in LMICs, the conceptual frameworks behind the use of these tools in other cultural contexts are not always carefully examined. In order to have valid data across cultures, we should aim to adapt and validate tools before use. Provision of processes to validate tools across LMIC settings would be beneficial.

Internalizing problems are characterized by anxiety, depressed mood, social withdrawal, and somatic complaints. Childhood internalizing problems are a concerning mental health issue due to their continuity into adolescence and associated functional impairment. This study focused on studying the relation between affective temperaments and internalizing problems, in a sample of adolescents in the community. There has been very little research in mental health problems in children and adolescents in Kosovo.The aim of this study was to identify the link between affective temperaments with youth psychopathology, by measuring both temperament with Temperament Evaluation of Memphis, Pisa, Paris and San Diego (TEMPS) and youth psychopathology with Youth Self Report (YSR) in the Kosovo sample. Our study found that depressive, cyclothymic, anxious, and irritiative temperaments were more displayed in female respondents. Meanwhile, hyperthermic temperament was not found to be reported as interrelated to gender.However, gender related differences were significant on the YSR scales, with female respondents reporting higher values on those scales. The study found that there is a significant difference between the groups with normal scores on Depressive Temperament with group with high scores for all scales of Internalizing problems. More scores in Depressive Temperament more scores in Internalizing Scales.The same tendencies were found for Cyclothimic Temperament and Anxious temperament. Hyperthermic temperament was not found to have significant effect on Anxious/ Depressed, Withdrawn, Somatic Complaints, and Social Problem. Irritative temperament was found to have significant effect only in Anxious/ Depressed F (2) = 13.1, p<.01, η2= .03.The higher scores in Anxious/ Depressed scores were found in the group with high scores in Irritative Temperament. The study concluded that temperament may only be one of several factors contributing to the development of psychopathology. This study found that the temperament, which is influenced by environment and biology, is a significant predictor for internalizing problems.
