Complete mesocolic excision with D3 lymph node dissection in laparoscopic colectomy for stages II and III colon cancer: long-term oncologic outcomes in 168 patients

Abstract: Standardization of laparoscopic CME and D3 lymphadenectomy is expedient. The technique is associated with acceptable morbidity and provides excellent oncologic outcomes for stage II and stage III colon cancer. A longer follow-up is needed to validate the enhancement of oncological outcome related to this surgical concept.

“…As expected, the operation time was significantly longer in the LAC group. There were 3 patients in the LAC group and 13 patients in the Open group who had postoperative complications, but there were no severe cardiovascular and pulmonary disorders in both groups, which suggests that LAC is a safe technique for oldest-old patients, as previously reported [15,16]. …”

“…Для сравнения: в исследовании J. W. Shin и соавт. общая частота ослож-нений была ниже -17,8 %, однако у 5,9 % пациентов была отмечена несостоятельность межкишечных ана-стомозов [21]. Данное осложнение в нашей исследуе-мой группе не встречалось.…”

“…Moreover, the feasibility and technical value of laparoscopic complete mesocolic excision with central vascular ligation via a medial approach is now established [9,10,11,12]. …”

“…The procedure is also oncologically safe for treating patients with colon cancer [3,4]. Complete mesocolic excision with central vascular ligation has been found to standardize colon cancer surgery, reduce local recurrence, and improve long-term survival [5,6], and the feasibility and safety of complete mesocolic excision in open and laparoscopic surgery have been demonstrated [7,8,9,10,11,12]. …”
