Long‐term Intrathecal Baclofen: Outcomes After More than 10 Years of Treatment

Abstract: The subjects reported low levels of pain, moderate levels of life satisfaction, normal levels of sleepiness, low-to-moderate levels of fatigue, infrequent spasms at mild-to-moderate severity, and high levels of satisfaction. The efficacy and favorable adverse effect profile of ITB therapy was sustained in this cohort of subjects with more than a decade of treatment.

“…There were no reported infective complications, early pump revisions or explantations. The most common side effects were baclofen-related: 3 (60.0%) out of the fi ve patients had transient hypotension immediately after ITB implantation, while patient 2 had excessive somnolence during a routine ITB refi ll ( [18][19][20][21][22][23] Despite intensive inpatient rehabilitation and follow-up after ITB pump implantation, the reduction in limb spasticity was not accompanied by a corresponding signifi cant increase in independent locomotor abilities or self-care. The only patient in this series who experienced reduced functional disability had central paraplegia sparing his upper limbs, intact cognition and motivation, which could have positively impacted rehabilitation gains.…”

“…The long-term efficacy and benefits of the ITB treatment on spasticity, pain, and spasm frequency has been investigated in several studies [36, 37]. To our knowledge, this is the first open study investigating the effectiveness of ITB treatment in patients with inherited ataxia.…”

“…Baclofen, the clinically approved GABA-B agonist, has potent stereospecific suppressive effects upon motor tone [253] and is widely used clinically to regulate motor tone in the case of spasticity [254][255][256]; but it has also been implicated in having suppressive effects upon nociceptive processing after intrathecal delivery in preclinical models of facilitated processing [217] and hyperpathia in mono and polyneuropathies and spinal injury [220][221][257][258][259] and in humans [260,261].…”
