Objetivos: Conocer la capacidad diagnóstica de las diferentes pruebas de imagen empleadas en nuestro centro. Valorar si la distribución de las diferentes variables que definen la tumoración es la misma en los tumores con y sin recidiva.Material y método: Se valoraron 54 casos de tumor de células gigantes de la vaina tendinosa de la mano. En todos se realizó una radiografía, en 19 ecografía y en 7 resonancia magnética. Los datos analizados fueron: sexo, edad, antecedente traumático, dedo afecto, localización, resultado de las pruebas de imagen, características anatomopatológicas de la tumoración (tamaño y número de nódulos), y presencia o no de recidiva.Resultados: Se objetivó 1,7 mujeres por cada varón. La mano derecha y el segundo dedo fueron las localizaciones más frecuentes. Ninguna radiografía fue diagnóstica; la ecografía tuvo un valor predictivo positivo de 36,8% y la resonancia magnética de 71,4%. Hubo 6 casos de recidiva, sin relación estadísticamente significativa entre la recidiva y el tamaño, el número de nódulos o la artrosis.Conclusión: La resonancia magnética tiene una capacidad diagnóstica superior a la ecografía, y esta, superior a la radiografía simple. No se ha encontrado ninguna asociación estadísticamente significativa entre la recidiva de la lesión y los factores estudiados.

PurposeThe purpose of this study was to prospectively evaluate the detectability and usefulness of automated whole breast ultrasound (AWUS) and to compare it with handheld breast ultrasound (HHUS) in cases with suspicious microcalcifications identified by mammography.MethodsForty-two patients with 43 suspicious microcalcifications (25 malignant and 18 benign) detected by mammography underwent AWUS, HHUS, and histol-ogic examination. With knowledge of the mammographic findings, HHUS was performed to assess the visibility of the microcalcifications and the presence of associated masses or ductal changes. Two radiologists reviewed the AWUS images in consensus using the same methods employed for HHUS. Detectability of AWUS was compared with that of HHUS and was correlated with histologic and mammographic findings.ResultsOf the 43 lesions, 32 (74.4%) were detectable by AWUS and 31 (72.1%) by HHUS. No significant differences in sensitivity were found between the two methods (p=0.998). AWUS detected 96% (24/25) of malignant microcalcifications and 44.4% (8/18) of benign microcalcifications. AWUS was more successful in the detection of malignant vs. benign lesions (96.0% vs. 44.4%, p=0.002), lesions >10 mm vs. ≤10 mm in size (86.7% [26/30] vs. 46.2% [6/13], p=0.009), lesions with a fine pleomorphic or linear shape vs. a round or amorphous or coarse heterogeneous shape (94.7% [18/19] vs. 58.3% [14/24], p=0.021), and lesions associated with a mass or architectural distortion vs. without obvious changes on mammography (100% [19/19] vs. 54.2% [13/24], p=0.022).ConclusionDetectability of AWUS was comparable to that of HHUS in cases where suspicious microcalcifications were identified on mammography. Therefore, AWUS might be helpful in the performance of ultrasound-guided percutaneous procedures for highly suspicious microcalcifications.

Breast density is a strong predictor of the failure of mammography screening to detect breast cancer and is a strong predictor of the risk of developing breast cancer. The many imaging options that are now available for imaging dense breasts show great promise, but there is still the question of determining which women are "dense" and what imaging modality is suitable for individual women. To date, mammographic breast density has been classified according to the Breast Imaging-Reporting and Data System (BI-RADS) categories from visual assessment, but this is known to be very subjective. Despite many research reports, the authors believe there has been a lack of physics-led and evidence-based arguments about what breast density actually is, how it should be measured, and how it should be used. In this paper, the authors attempt to start correcting this situation by reviewing the history of breast density research and the debates generated by the advocacy movement. The authors review the development of breast density estimation from pattern analysis to area-based analysis, and the current automated volumetric breast density (VBD) analysis. This is followed by a discussion on seeking the ground truth of VBD and mapping volumetric methods to BI-RADS density categories. The authors expect great improvement in VBD measurements that will satisfy the needs of radiologists, epidemiologists, surgeons, and physicists. The authors believe that they are now witnessing a paradigm shift toward personalized breast screening, which is going to see many more cancers being detected early, with the use of automated density measurement tools as an important component.
