2022 Esc Guidelines for the Management of Patients with Ventricular Arrhythmias and the Prevention of Sudden Cardiac Death: What is New?

Abstract: The review presents new indications to help with diagnosis and treatment of ventricular arrhythmia (VA) in patients with various etiologies of rhythm disturbances, including patients with coronary artery disease, cardiomyopathies, channelopathies, inflammatory heart disease, neuromuscular disease, and congenital heart defects. Algorithms for diagnostic evaluation at first presentation with VAs in patients without known cardiac disease are given.

“…Current guidelines [ 83 ] recommend the use of immunosuppressive therapy in selected cases of virus-negative biopsy-proven myocarditis, on top of OMT that should be targeted on the specific patient’s phenotype, e.g., anti-HF drugs in case of left ventricular dysfunction or antiarrhythmic medical therapy in case of hemodynamically stable sustained ventricular arrhythmias [ 132 ].…”

“…The diagnosis of ACM is complex and requires searching and assessing certain criteria. Although the recommendations contain a reference in Guidelines 2022 to the publication of Corrado et al (2020) [28], the criteria for diagnosing ACM are not the 2020 Padua criteria but the previous 2010 criteria [32]. As an advantage, the Padua criteria provide an algorithm for diagnosing not only the right ventricular but also the LV process.…”

“…Неустойчивая и устойчивая ЖТ -частая находка у пациентов, страдающих ГКМП . В связи с высоким риском ВСС при этой патологии требуются стратификация степени риска и определение показаний к установке ИКД [1,3,11,12] . Наличие у нашего пациента устойчивых ЖТ, сопровождающихся нарушениями гемодинамики, потребовало установки ИКД в 2007 году и замен в 2012 и 2020 годах в связи с истощением заряда батареи .…”
