2017 update on pain management in patients with chronic kidney disease

Abstract: The prevalence of pain has been reported to be >60–70% among patients with advanced and end-stage kidney disease. Although the underlying etiologies of pain may vary, pain per se has been linked to lower quality of life and depression. The latter is of great concern given its known association with reduced survival among patients with end-stage kidney disease. We herein discuss and update the management of pain in patients with chronic kidney disease with and without requirement for renal replacement therapy w… Show more

“…Though while a degree of attentiveness was suggested, there seems to be a variance of recommendations in those with an eGFR <45 mL/min. This is unsurprising given the lack of data to support clinical decision making (Pham et al, ) and warrants further research for guidance.…”

“…While it is likely topical NSAIDs are a safer option than oral NSAIDs for prevention of renovascular injury, especially in those with chronic kidney disease (CKD), there is a paucity of data to support this (Pham et al, ). Guidelines are almost non‐existent, the 2012 Kidney Disease improving Global Outcomes guidelines (KDIGO, ) suggest discontinuation of all NSAIDs in those with an estimated Glomerular Filtration Rate (eGFR) <30 mL/min (topical NSAIDs are not specifically mentioned), Pham et al () in their recommendations of pain management in CKD, suggest caution in those with an eGFR <30 mL/min.…”

“…Another consideration is that fentanyl is a more lipophilic agent and distributes into fatty tissues, giving it a longer half‐life in patients with large fat stores 12 . Neonates are also at particularly high risk for respiratory depression due to immature hepatic enzymes and decreased hypoxic respiratory drive 13 . Pain control in the pediatric population is a perplexing patient‐specific problem, and management has been further complicated due to the national shortages of many of the IV opioid pain management options…”

“…3 An estimated 60% to 70% of patients with chronic kidney disease and ESRD have chronic pain. 6 The stress from living in homeless shelters, poor sleeping conditions, inability to afford medications, and missed appointments exacerbate pain in patients who are homeless, which can lead to reliance on street drugs and an increased risk of overdose. 7 The myriad management challenges and logistical barriers lead to missed dialysis sessions and promote heavy reliance on emergency-only dialysis.…”
