18F-FDG Versus 68Ga-FAPI PET/CT in Visualizing Primary Hepatic Extranodal Marginal Zone Lymphoma of Mucosa-Associated Lymphoid Tissue

Abstract: The prevalence of primary hepatic mucosa-associated lymphoid tissue lymphomas is extremely low. In this case, we present 18 F-FDG and 68 Ga-FAPI PET/CT findings in a 76-year-old man with pathologically confirmed primary hepatic extranodal marginal zone lymphoma of mucosaassociated lymphoid tissue. 68 Ga-FAPI PET/CT showed a better tumor-tobackground ratio than 18 F-FDG PET/CT in the hepatic lesion because there is little 68 Ga-FAPI uptake in the liver parenchyma. This case indicates that 68 Ga-FAPI PET/CT may… Show more

“…6 Recent studies suggested that 68 Ga-FAPI PET/CT demonstrated superiority over 18 F-FDG PET/CT in the evaluation of various cancer types, especially in gastrointestinal cancer. [7][8][9] This case demonstrated that when compared with 18 F-FDG, 68 Ga-FAPI-46 PET/CT detected liver metastases and more metastatic lymph nodes with higher sensitivity and better tumor-to-background contrast. Therefore, 68 Ga-FAPI-46 PET/CT may be advantageous over 18…”

“…Therefore, early diagnosis and accurate staging are circuital in the management of patients with SMARCA4-deficient undifferentiated carcinoma 6. “…Many recent studies have demonstrated that 68 Ga-FAPI is superior to 18 F-FDG in tumor detection in hepatic diseases. [5][6][7][8] At the same time, these hepatic diseases show similar PET/CT manifestations, which warn us that we should pay attention to differential diagnosis. [9][10][11] Remarkably, in this case, multiple lesions in the liver exhibited FAPI uptake at different levels, which may reflect tumor heterogeneity of the disease.…”

“…Based on this characteristic, inflammatorymyofibroblastoma tumor could demonstrate high 68 Ga-FAPI uptake, 4 consistent with what was shown in this case. Many recent studies have demonstrated that 68 Ga-FAPI is superior to 18 F-FDG in tumor detection in hepatic diseases 5–8 . At the same time, these hepatic diseases show similar PET/CT manifestations, which warn us that we should pay attention to differential diagnosis 9–11 .…”
