18F-FDG PET as a Monitoring Tool to Assess Treatment Response in Bilateral Adrenal Histoplasmosis

Abstract: A 60-year-old woman with clinical and biochemical features suggestive of adrenal insufficiency was found to have bilateral adrenal masses on CT scan and was subjected to F-FDG PET scan. The scan showed hypermetabolic mediastinal nodes in addition to intense tracer uptake in bilateral adrenal masses. CT-guided adrenal biopsy grew Histoplasma capsulatum on Sabouraud dextrose agar culture. A second F-FDG PET scan after 6 months of appropriate antifungal therapy showed a significant decrease in intensity of uptake… Show more

“…However, due to persistent symptoms, she was subjected to adrenal biopsy and diagnosed to have histoplasmosis. With antifungal medications (IV amphotericin B followed by oral itraconazole), there was regression of adrenal masses (right 4.4cm, left 3.8cm) at 19-month follow-up (11). …”

“…Modern imaging modalities can provide excellent resolution for visualization of infectious processes, but imaging is usually limited to detecting advanced disease states. There are isolated case reports of diagnosing disseminated histoplasmosis with 18-Fluorodeoxyglucose positron emission tomography ( 18 F-FDG-PET/CT) [38][39][40][41]. In areas where Histoplasma is endemic, certain findings, such as adrenal masses with intense FGD uptake, should raise suspicion for histoplasmosis but does not provide a definitive diagnosis of histoplasmosis [41].…”
