Purpose: This study aimed at assessment of respiratory muscle performance (RMP), functional capacity (FC), feeling of fatigue, and quality of life (QOL) in liver transplant candidates and to correlate these variables with each other and with the Model for End-Stage Liver Disease (MELD). Methods: A prospective study carried out from January/2008 to June/2009 including 130 liver transplant candidates on waiting–list at the São Paulo Health Transplant System, followed at the Unit of Liver Transplantation - Unicamp with cirrhosis of any etiology, independent of gender, age, and degree of liver function impairment as classified by the MELD score. Respiratory muscle performance was determined by respiratory muscle strength (RMS - PIMAX and PEMAX), vital capacity (VC), and FC measured during six-minute walk tests (6MWT). Fatigue was evaluated using the Fatigue Severity Scale (FSS), and QOL was assessed using the SF-36 questionnaire. Results: Expressed as the mean SD and percentage of the predicted value: MELD=16+4, VC=4.0+0.9L - (97%+16%), PIMAX=77+28cmH2O -(71+25%), PEMAX=76+26cmH2O (76+26%), 6MWT=453.9+81.2m (73+13%), FSS=34+17, and compromised QOL, particularly in physical (48+41) and emotional (57+41) aspects, vitality (58+26) and general state (58+24). Predicted VC was negatively correlated with MELD (p=0.0034) and positively correlated with RMS and 6MWT (p=0.0217). Greater distances in the 6MWT were associated with a higher RMS and QOL and less fatigue. Greater FSS scores resulted in lower QOL scores. Conclusion: A decrease in VC was related to the severity of liver disease and a decrease in RMS and FC, which, in turn, correlated with greater fatigue and lower QOL.

Objetivo: Avaliar o efeito das medidas pré-operatórias das pressões inspiratória máxima (PImáx) e expiratória máxima (PEmáx) no resultado do transplante de fígado (Tx). Métodos: Foram estudados retrospectivamente 228 pacientes submetidos a primeiro Tx eletivo. Os pacientes foram classificados conforme a ocorrência de valores absolutos de pressão respiratória menores ou iguais a 50 cm H2O. As variáveis estudadas foram: tempo de ventilação mecânica pós-operatória, necessidade de re-intubação orotraqueal ou de ventilação mecânica não-invasiva, tempo de internação e sobrevida. Resultado: Os resultados mostraram que os valores observados de PImáx e PEmáx estavam abaixo de 50 cm H2O em 19,7% (45/228) e 14,5% (33/228) dos pacientes, respectivamente. A freqüência de óbito até seis meses após o transplante foi de 26/183 (14,2%) nos pacientes com PImáx > 50 cm H2O e de 15/45 (33,3%) nos pacientes com PImáx mais baixa (p=0,003). A sobrevida de 1, 3 e 5 anos foi 84%, 77% e 71% no grupo com PImáx > 50 cm H2O e 57%, 50% e 50% no grupo com PImáx mais baixa (p=0,0024). Em relação à PEmáx, essas probabilidades foram 80%, 74% e 69% no grupo com valores maiores que 50 cm H2O e 66%, 59% e 51% nos pacientes com força expiratória menor (p=0,1039). Não houve diferença estatisticamente significante em relação às demais variáveis analisadas. Conclusão: Pacientes com PImáx baixa apresentam maior mortalidade após o Tx. Entretanto, não foram encontrados efeitos estatisticamente significantes da medida pré-operatória da força da musculatura respiratória nas variáveis de resposta mais diretamente relacionadas com alterações respiratórias.

BACKGROUND: Liver cirrhosis (LC) causes several musculoskeletal changes. OBJECTIVE: To test the hypothesis that the peripheral and inspiratory muscle endurance are reduced in patients with liver cirrhosis. METHODS: Twenty-one patients with LC (LC group; 61±14 years) and 18 age-matched subjects (control group; 56±17 years) had accepted to participate in this cross-sectional observational study. To assess peripheral muscle endurance, all volunteers performed a rhythmic handgrip exercise at 45% of their maximum voluntary contraction. A metronome was used to control the contraction-relaxation cycles at 60/min. The inspiratory muscle endurance was assessed using PowerBreath®. Participants underwent inspiratory muscle exercise at 60% of their maximal inspiratory muscle strength. The time until failure characterized the muscle endurance for both handgrip and inspiratory muscle exercises. Additionally, the quality of life of the participants was assessed. RESULTS: The muscle endurance was lower in the LC group when compared to the control group for both handgrip (67 vs 130 s, P<0.001) and inspiratory muscle exercises (40 vs 114 s, P<0.001). The peripheral muscle endurance was directly correlated with the total quality of life score (r=0.439, P=0.01) and to the following domains: fatigue (r=0.378 e P=0.030), activity (r=0.583, P=0.001), systemic symptoms (r=0.316, P=0.073) and preoccupation (r=0.370, P=0.034). The inspiratory muscle endurance was inversely correlated with the total quality of life score (r=0.573, P=0.001) and the following domains: fatigue (r=0.503, P=0.002), activity (r=0.464, P=0.004), systemic symptoms (r=0.472, P=0.004), abdominal symptoms (r=0.461, P=0.005), emotional function (r=0.387, P=0.02) and preoccupation (r=0.519, P=0.001). CONCLUSION: Both peripheral and inspiratory muscle endurance were lower in LC patients when compared to the control group.
