Quality of Life after Rhinoplasty: A Prospective Study

As our previous studies have shown, cosmetic surgery has a positive correlation with postoperative well-being. The aim of this study was to prospectively examine the postoperative changes in quality of life (QoL) after a rhinoplasty. Thirty-four patients who underwent septorhinoplasty performed by a single surgeon from July 2015 to October 2018 reported in indication-specific self-developed and different validated questionnaires (FLZM or Fragen zur Lebenszufriedenheit Module, Freiburg Personality Inventor, Rosenberg self-esteem scale, Functional Rhinoplasty Outcome Inventory 17 [FROI-17], and Glasgow Benefit Inventory [GBI]) on the status of their QoL preoperatively (T0) and 6 months' follow-up (T1). Our goal was to assess the difference in psyche and self-esteem and to get objective insights into the effect of the operation. Significant improvements in QoL in terms of general module, health, and appearance were noted. The general part of the FLZM showed increasing T1 values in the sum scores (p = 0.005). With regard to the item “health,” T1 was better than the norm data (p = 0.003). The statistically significant improvement for the item nose appearance (p < 0.0001) after operation and T1 versus reference data (p < 0.010) should be highlighted. The subjective patient ratings showed statistically significant T1 improvements for all items of the FROI-17: overall nose (p < 0.0001), nasal function (p = 0.001), general/further symptoms (p = 0.006), and confidence increased by aesthetic changes (p < 0.0001). Furthermore, the GBI score shows an improved QoL after rhinoplasty (p < 0.0001). Based on the assessment of a variety of disease- and nondisease-specific validated questionnaires, numerous improvements in the QoL of the patients were observed. Therefore, we support septorhinoplasty as a meaningful procedure regarding QoL improvement. The level of evidence is Level II prospective cohort study.

An Artificial Intelligence Tool for Image Simulation in Rhinoplasty

During rhinoplasty consultations, surgeons typically create a computer simulation of the expected result. An artificial intelligence model (AIM) can learn a surgeon's style and criteria and generate the simulation automatically. The objective of this study is to determine if an AIM is capable of imitating a surgeon's criteria to generate simulated images of an aesthetic rhinoplasty surgery. This is a cross-sectional survey study of resident and specialist doctors in otolaryngology conducted in the month of November 2019 during a rhinoplasty conference. Sequential images of rhinoplasty simulations created by a surgeon and by an AIM were shown at random. Participants used a seven-point Likert scale to evaluate their level of agreement with the simulation images they were shown, with 1 indicating total disagreement and 7 total agreement. Ninety-seven of 122 doctors agreed to participate in the survey. The median level of agreement between the participant and the surgeon was 6 (interquartile range or IQR 5–7); between the participant and the AIM it was 5 (IQR 4–6), p-value < 0.0001. The evaluators were in total or partial agreement with the results of the AIM's simulation 68.4% of the time (95% confidence interval or CI 64.9–71.7). They were in total or partial agreement with the surgeon's simulation 77.3% of the time (95% CI 74.2–80.3). An AIM can emulate a surgeon's aesthetic criteria to generate a computer-simulated image of rhinoplasty. This can allow patients to have a realistic approximation of the possible results of a rhinoplasty ahead of an in-person consultation. The level of evidence of the study is 4.

IMPORTANCE To date, the impact of rhinoplasty surgery on social perceptions has not been quantified. OBJECTIVE To measure the association of rhinoplasty with observer-graded perceived attractiveness, success, and overall health. DESIGN, SETTING, AND PARTICIPANTS In a web-based survey, blinded casual observers viewed independent images of 13 unique patient faces before or after rhinoplasty. Delphi method was used to select standardized patient images, confirming appropriate patient candidacy and overall surgical effect. Observers rated the attractiveness, perceived success, and perceived overall health for each patient image. Facial perception questions were answered on a visual analog scale from 0 to 100, where higher scores corresponded to more positive responses. A multivariate mixed-effects regression model was used to determine the effect of rhinoplasty while accounting for observer biases. To further characterize the effect of rhinoplasty, estimated ordinal rank change was calculated for each domain. MAIN OUTCOMES AND MEASURES The primary objective was to measure the effect of rhinoplasty on observer-graded perceived attractiveness, success, and overall health. RESULTS A total of 473 observers (mean age, 29 years [range, 18-73 years]; 305 [70.8%] were female) successfully completed the survey. On multivariate regression, patients after rhinoplasty were rated as significantly more attractive (rhinoplasty effect, 6.26; 95% CI, 5.10-7.41), more successful (rhinoplasty effect, 3.24; 95% CI, 2.32-4.17), and overall healthier (rhinoplasty effect, 3.78; 95% CI, 2.79-4.81). The ordinal rank change for an average individual's perceived attractiveness, success, and overall health was a positive shift of 14, 9, and 10 out of 100 rank positions, respectively. CONCLUSIONS AND RELEVANCE As perceived by casual observers, rhinoplasty surgery was associatedwith perceptions that in patients appeared significantly more attractive, more successful, and healthier. These results suggest patients undergoing rhinoplasty may derive a multifaceted benefit when partaking in social interactions. Furthermore, these results facilitate improved patient discussions aiming to provide more precise surgical expectations with an understanding that these results represent optimal outcomes.
