Background Antrochoanal or Killian polyp is a rare polyp of the maxillary sinus in adults. It is generally unilateral, implanted on the lateral wall of the maxillary sinus upon the uncinate process. Bilateral cases are very rare; only 14 cases have been cited from 1980 to 2020. Case presentation Our patient presented with a complete bilateral nasal obstruction, this symptom persisted for more than 10 years, with a perceived prolapse in his throat in the last 4 months. Anterior rhinoscopy showed a polyp that took up the totality of his nasal vestibule. Posterior rhinoscopy showed a polyp bulging behind the uvula. The challenge in this case is related to diagnosis decision; conditions with the same clinical presentation are several. Diagnosis was established through the clinical, radiological, and endoscopic aspects, and confirmed by histological findings. Management was obtained through functional endoscopic sinus surgery; no recurrence was noted in the 12-month follow-up, and clinical signs had completely subsided. Conclusion Diagnosis in bilateral forms of Killian polyp is perplexing; however, its management is generally simple, with excellent outcome. We report the case of the 15th patient along with a systematic literature review discussing bilateral Killian polyps, illustrated by a rare case of enormous bilateral antrochoanal polyps.

Antrochonal polyp (ACP) is a benign, rare, and generally unilateral tumor which originates from the maxillary sinus mucosa. Bilateral ACP is extremely rare. Only few cases have been documented in the literature until 2018. The authors report the case of a 44-year-old women, who presented with a bilateral progressive nasal obstruction for the past 2 years, slight headache and a decreased sense of smell has been started in last four months. Nasal endoscopy revealed pale polypoidal masses in nasal cavities, arising from each middle meatus and extending to the nasopharynx. Computed tomography of the paranasal sinuses revealed the presence of soft-tissue masses in the maxillary sinuses, passing through the maxillary ostium, and extending into the corresponding nasal cavities, and posteriorly upto the nasopharynx. The other sinuses were normally aerated. The tumors were removed surgically with a nasal endoscopy technique. Histopathology examination the two lesions revealed benign inflammatory nasal polyps.

Introduction and importance: Antrochoanal polyp (ACP) is a well-known benign sinonasal polyp first described in the early 20th century. ACP mostly presents as a unilateral mass and is treated solely with surgical excision. Case presentation: We report a rare case of a middle-aged man presenting with nasal obstruction, rhinorrhea, and sleeping disturbances, eventually diagnosed with bilateral ACPs. After confirming the diagnosis with imaging and biopsy studies, the patient was treated conservatively, with marked improvements in his symptoms during 2–3 months of regular follow-ups. A review of the relevant literature regarding the presentation, diagnosis, and outcome of this rare entity is presented, highlighting its controversial etiopathogenesis. Clinical discussion: Presenting symptoms of ACP in most cases is unilateral progressive nasal obstruction. The occurrence of ACP bilaterally is rarely encountered in clinical practice. Diagnosis is mainly clinical and is achievable via nasal endoscopic examination and supported by computed tomography imaging. Treatment remains to be surgical, with 2 years of regular follow-ups being advised to detect any recurrence. Conclusion: This case report adds to the scarce data pool on bilateral ACPs and highlights the necessity of prudent and timely diagnosis of this uncommon entity to avoid unnecessary investigations and lengthy medical or surgical treatment. Additionally, a trial of medical therapy may provide symptomatic relief for patients who do not qualify for surgery.
