Abstract: By using cautery, cold instrumentation and laser at different steps of endoscopic DCR we were able to achieve a success rate of around 94% with this composite technique as compared to 83.3% in conventional endoscopic DCR surgery.

“…Amit Pal Singh et al found in 15 patient success rate in conventional endonasal DCR has found 83.3%, and intra-operative and post-operative excessive bleeding found in 2 patients (13.2%). 10 In our study we found in conventional endonasal DCR, success rate was 93% and intra-operative and post-operative excessive bleeding was found in 33.3%. In powered endoscopic DCR excessive intra-operative and post-operative bleeding was found in 10% only.…”

“…Surgical ostium and lacrimal fistula can be obstructed because mucosal flaps cannot be created in laser DCR. Recently, surgeons have tried methods such as the use of low energy lasers [ 10 , 12 ], early cleaning of the surgical ostium [ 17 ], surgical ostium mitomycin C (MMC) application [ 15 , 18 ], and laser-assisted dacryocystorhinostomy with endonasal augmentation with punch [ 9 , 19 , 20 ] to decrease fibrous scar formation. Along with photothermal damage caused by the laser, increased fibroblastic activity in the nasal mucosa [ 21 ], scar formation and narrowing in the ostium can be observed.…”
