An update on pathology of IgA nephropathy

Abstract: IgA Nephropathy (IgAN) is the commonest of the glomerular diseases in the world. Its progression rate of 30-40% of the cases em 20-30 years makes IgAN an important healthcare issue in Nephrology. Diagnosis of IgAN depends on biopsy findings, particularly at immunofluorescence microscopy. The frequence of IgAN diagnosis is variable in different populations and depends on screening and biopsy indication policies. IgAN pathogenesis is considered multifactorial; its primordial defect is the production of galactosi… Show more

“…Polymeric IgA1 deposited in the mesangial of kidney leads to IgA nephropathy (IgAN). This is one of the most common cause of glomerulonephritis all around the world . Studies showed that IgAN was associated with an increase in B cells number and the incompletely galactosylation of O‐glycans in IgA1.…”

“…IgAN is characterized by codominance or dominance of polymeric IgA1 in mesangial deposition that originates from circulating IgA1‐containing polymeric IgA1 or immune complexes . However, IgAN was considered as one complex multifactorial disease, immunology and genetics mechanisms were proved to play critical roles in the development and pathogenesis of IgAN . Growing evidences suggested that B lymphocytes were increased in cases with high concentration of the IgA in the serum .…”

“…[6][7][8] Growing evidences suggested that B lymphocytes were increased in cases with high concentration of the IgA in the serum. 6,9 IgA1 in the serum is abnormally O-glycosylated, which causes IgA1 mesangial deposition and glomerular injury developing into IgAN. 10,11 MicroRNAs (miRNAs) are a group of single-stranded, noncoding, and short (21 to 23 nucleotides) RNA molecules that regulate gene expression through binding to 3′UTR of mRNAs of the protein coding genes.…”

“…The combined maximal score of this classification (MEST score) was 5 points (M1+E1+S1+T2) and renal lesions with a sum score of two or higher were accepted as having an independent risk factor for progression to ESRD for IgAN patients [7]. In addition, each of these 4 separate lesions was proposed as having an independent value as a risk factor for progression to ESRD [8]. S1 lesion was found to be associated with the presence of higher proteinuria levels and better response to immunosuppressive treatment [9].…”
