Herein we describe two cases of persistent mydriasis after gonioscopy-assisted transluminal trabeculotomy (GATT) for open-angle glaucoma (OAG). Both surgeries were uneventful, but the patients experienced postoperative hyphema and IOP elevation. They then developed persistent fixed and dilated pupils resistant to pilocarpine that led to intolerable photosensitivity and glare. An iris cerclage pupilloplasty was performed with adequate relief of symptoms in one case.
