Die Auswirkung von Zykloplegie auf das optische Niedrigkohärenz-Reflektometrie-Biometer ZUSAMMENFASSUNG Zweck: Das Ziel dieser Studie ist es, die biometrischen Messungen und verschiedene Formeln zur Berechnung der Intraokularlinsenstärke unter Verwendung des optischen Low-Coherence-Reflektometrie-Biometers Lenstar LS 900 vor und nach der Induktion von Zykloplegie bei Erwachsenen zu vergleichen Material und Methoden: In diese Querschnittsstudie wurden 168 Augen von 168 gesunden Freiwilligen im Alter von 40–86 Jahren (59,22 ± 11,57) eingeschlossen. Biometrische Messungen, einschließlich Achsenlänge (AL), Vorderkammertiefe (ACD), Keratometrie (K1, K2) und Weiß-zu-Weiß (WTW) wurden mit einem optischen Biometriegerät Lenstar LS 900 vor und nach Induktion einer Zykloplegie mit Cyclopentolat %1 verglichen. Die Stärke der Intraokularlinse (IOL) wurde auch unter Verwendung von sechs verschiedenen Formeln (Barrett Universal II, Haigis, SRK/T, Hoffer Q, Holladay und SRK-II) für die Intraokularlinse AcrySof MA60AC vor und nach Induktion der Zykloplegie verglichen. Ergebnisse: Es gab keine statistisch signifikanten Unterschiede in Achslänge, Keratometrie (K1 und K2) oder Weiß-zu-Weiß-Messungen vor und nach Induktion der Zykloplegie. Es gab nur einen signifikanten Anstieg der ACD aus den biometrischen Parametern nach der Induktion der Zykloplegie (P < 0,05). Trotz dieser Änderung gab es keine signifikanten Änderungen bei den Berechnungen der IOL-Stärke unter Verwendung der sechs verschiedenen Formeln vor und nach der Induktion der Zykloplegie. Schlussfolgerungen: Diese Studie zeigte, dass IOL-Stärkemessungen mit dem Lenstar LS 900 nach Zykloplegie durchgeführt werden können. Abstract Purpose: The aim of this study is to compare the biometric measurements and different intraocular lens power calculation formulas by using Lenstar LS 900 optical low coherence reflectometry biometer device before and after induction of cycloplegia in adult population Materials and Methods: In this cross-sectional study, 168 eyes of 168 healthy volunteers aged 40-86 years (59.22±11.57) were included. Biometric measurements, including axial length (AL), anterior chamber depth (ACD), keratometry (K1, K2) and white-to-white (WTW) were compared using a Lenstar LS 900 optical biometry device before and after induction of cycloplegia with cyclopentolate %1. The intraocular lens (IOL) power was also compared using six different formulas (Barrett Universal II, Haigis, SRK/T, Hoffer Q, Holladay and SRK-II) for the AcrySof MA60AC intraocular lens before and after induction of cycloplegia. Results: There were no statistically significant differences in axial length, keratometry (K1 and K2) or white-to-white measurements before and after induction of cycloplegia. There was a significant increase only in ACD from the biometric parameters after the induction of cycloplegia (P<0.05). Despite this change, there were no significant changes in IOL power calculations using the six different formulas before and after the induction of cycloplegia. Conclusions: This study demonstrated that IOL power measurements using the Lenstar LS 900 can be performed after cycloplegia.

The accuracy of using vergence formula to screen myopia in children: a cross-sectional study

ObjectiveTo evaluate the accuracy of using the vergence formula to screen myopia in children and adolescents.MethodsThis was a cross-sectional study conducted between December 2022 and May 2023 at the ophthalmology clinic of Beijing Tongren Hospital. A total of 336 children aged 6 to 12 years with refractive errors were selected according to the inclusion criteria. Biometric measurements, including axial length, corneal thickness, anterior chamber depth, corneal curvature, and lens thickness, were obtained using a biometer. The Calculated spherical equivalent (SE) was then calculated using the vergence formula. Cycloplegic refraction was performed after paralysis of the ciliary muscle, and the subjective SE was recorded. A diagnosis of myopia was made if the subjective SE was ≤ −0.50 diopters.ResultsThe AL/CR, subjective SE, and calculated SE were not normally distributed (p < 0.05). The AL/CR value was 3.08 (2.81, 3.27), the SE was −1.60 D (−6.00 D, 3.75 D), and the calculated SE was −1.42 D (−6.64 D, 5.73 D). There was no significant difference between the calculated SE and the SE (Z = −2.899, p = 0.004). The AL/CR value was negatively correlated with SE (r = −0.687, p < 0.01), and the calculated SE was positively correlated with SE (r = 0.827, p < 0.01). The area under the ROC curve for predicting myopia using AL/CR and calculated SE was 0.876 and 0.962, respectively, and the difference between the two was significant (p < 0.001). The sensitivity of AL/CR was 84.2%, the specificity was 70.6%, the accuracy was 82.1%, and the Youden index was 0.548. The sensitivity of calculated SE was 83.1%, the specificity was 100%, the accuracy was 85.7%, and the Youden index was 0.831.ConclusionThe vergence formula can be used to evaluate myopia in children and adolescents with relatively high accuracy without cycloplegic refraction.

High-precision biometry and accurate intraocular lens (IOL) power calculation have become essential components of cataract surgery. In clinical practice, IOL power calculation involves measuring parameters such as corneal power and axial length and then applying a power calculation formula. The importance of posterior corneal curvature in determining the true power of the cornea is increasingly being recognized, and newer investigative modalities that can estimate both the anterior and posterior corneal power are becoming the standard of care. Optical biometry, especially using swept-source biometers, with an accuracy of 0.01–0.02 mm, has become the state-of-the-art method in biometry. With the evolution of IOL formulas, the ultimate goal of achieving a given target refraction has also moved closer to accuracy. However, despite these technological efforts to standardize and calibrate methods of IOL power calculation, achieving a mean absolute error of zero for every patient undergoing cataract surgery may not be possible. This is due to inherent consistent bias and systematic errors in the measurement devices, IOL formulas, and the individual bias of the surgeon. Optimization and personalization of lens constants allow for the incorporation of these systematic errors as well as individual bias, thereby further improving IOL power prediction accuracy. Our review provides a comprehensive overview of parameters for accurate biometry, along with considerations to enhance IOL power prediction accuracy through optimization and personalization. We conducted a detailed search in PubMed and Google Scholar by using a combination of MeSH terms and specific keywords such as “ocular biometry,” “IOL power calculations,” “prediction accuracy of refractive outcome in cataract surgery,” “effective lens position,” “intraocular lens calculation formulas,” and “optimization of A-constants” to find relevant literature. We identified and analyzed 121 relevant articles, and their findings were included.
