Use of the Ishikawa diagram in a case-control analysis to assess the causes of a diffuse lamellar keratitis outbreak

Abstract: The Ishikawa diagram, like most quality tools, is a visualization and knowledge organization tool. This systematization allowed the investigators to thoroughly assess all the possible causes of DLK outbreak. A clear view of the entire surgical logistics permitted even more rigid management of the main factors involved in the process and, as a result, highlighted factors that deserved attention. The case-control analysis on every factor raised by the Ishikawa diagram indicated that the commonly suspected factor… Show more

“…From the basic categories, i.e., 5M + E, selected categories were chosen for the analysis, method, man, environment, management, and material, which were considered adequate for the analyzed problem. The Ishikawa diagram allowed us to identify the potential causes [23][24][25], choosing the main causes from among them. For the selected main causes, the analysis was performed with the 5Why method [26,27], which consisted of asking successive questions "why?".…”

“…Para a determinação dos pontos-chave do problema escolhido, optou-se por utilizar o diagrama de Ishikawa, que é uma ferramenta empregada para a visualização das possíveis causas de um determinado problema. Tal ferramenta facilita o reconhecimento das raízes do problema e possibilita o planejamento de ações mais assertivas e resolutivas (Lira et al, 2017).…”

“…The Ishikawa diagram is also known as a cause-effect diagram, fishbone diagram, or root cause analysis diagram, and was developed by Kaoru Ishikawa in the 1960s [63,64]. It helps to visualize a problem and categorize its root causes; it is considered as one of the seven basic quality management tools.…”
