Descriptive analysis of the type and design of contact lenses fitted according to keratoconus severity and morphology

Abstract: Monocurve RGPCL were most frequently fitted in patients with mild to moderate keratoconus and oval cones morphology, while bicurve lenses were more frequently fitted in patients with severe and advanced keratoconus. This was probably because bicurve lenses are more appropriate for round cones due to increased corneal asphericity.

“…Another basic parameter of a CL is its base curve. Attending to this detail, RGPCL fitted the range from standard monocurve, bicurve, or multicurve lenses [ 34 , 35 ] to reverse geometry lenses [ 36 ] or specific curvature designs such as those developed by Rose [ 37 ] or implemented in the IKone lenses [ 31 ].…”

“…Likewise, in early stages of keratoconus, the manifest refraction of these patients shows an increase in both myopia and irregular astigmatism, commonly ranging between 2.00 and 8.00 D, with absence of parallelism of keratometry mires with a mean corneal power under 52.00 D (Table 1) [7]. Although RGP lenses are still the best choice to improve visual performance in patients with moderate keratoconus, different available designs of RGP contact lenses and their proficiency of centration in proportion with corneal topography is challenging [18]. Our goal in this study was to show that each RGP contact lens reduced aberrations by different mechanisms.…”

“…Monocurve RGPs are used in mild to medium KC, and multicurve CLs are used in advanced KC. However, in some advanced KC, fitting of corneal RGPs may be more difficult and lens decentralization, dislocation, and disconfort may be encountered [33].…”

“…Modern HCLs are indicated when there is RGP intolerance or poor centralization, when an optimal RGP fit cannot be achieved, when there is reduced daily wearing time of RGP. They have also been shown to help improve vision after keratoplasty [33,50]. Since these lenses with central RGP function and have soft peripheral skirt, they provide comfort as well as correcting vision.…”
