Background: The laboratory tests most used by blood banks to diagnose anemia are the hemoglobin (Hb) and microhematocrit (Hct) tests, measured from capillary samples. Objective: To analyze the two capillary screening methods for pre-donation anemia by comparing their agreement in diagnosing anemia. Method: A cross-sectional study in a population of 15,521 blood donation candidates for whom information was available on Hb and Hct, performed from capillary blood samples. Hb was determined using the HemoCue® test and Hct by the centrifugation method. The Kappa coefficient was calculated to assess the agreement between the methods. Pearson’s correlation tests and gender-adjusted linear regression were used to assess the change in the response variable (Hb) as a function of the explanatory variable (Hct). Results: The majority of the study population were men (70.4%), aged between 18 and 44 years (72.1%), who declared themselves white or mixed skin color (85.6%), and had undergone at least 11 years of complete education (72.4%). The Kappa coefficient found was 92.7 and 99.2 for women and men, respectively. Pearson’s correlation showed a correlation coefficient of 0.98 and the linear regression graph showed an adequate relationship between the tests with R2 = 0.97. Conclusions: Comparing the Hb and Hct capillary tests, it was found that Hct can be safely used to screen for anemia in pre-blood donation.

Objective To compare the diagnostic performance of 10 mathematical formulae for identifying thalassemia trait in blood donors. Methods Compete blood counts were conducted on peripheral blood specimens using the UniCel DxH 800 hematology analyzer. Receiver operating characteristic curves were used to evaluate the diagnostic performance of each mathematical formula. Results In the 66 donors with thalassemia and 288 subjects with no thalassemia analyzed, donors with thalassemia trait had lower values for mean corpuscular volume and mean corpuscular hemoglobin than subjects without thalassemia donors (77 fL vs 86 fL [P < .001]; 25 pg vs 28 pg [P < .001]). The formula developed by Shine and Lal in 1977 showed the highest area under the curve value, namely, 0.9. At the cutoff value of <1812, this formula had maximum specificity of 82.35% and sensitivity of 89.58%. Conclusions Our data indicate that the Shine and Lal formula has remarkable diagnostic performance in identifying donors with underlying thalassemia trait.

Iron Supplements and Iron Status in Frequent Blood Donors

Background: Iron deficiency anemia is a public health problem worldwide, which can cause by nutrition deficiency, blood loss, and regular blood donation. This study aims to evaluate the hematological parameters and serum ferritin of Thai male blood donors.Methods: Eighty-nine male blood donors were divided into three groups according to their frequency of blood donation: a group of 30 cases for first time donors (group I), a group of 30 cases for donors who have donated 3-6 times (group II), and a group of 29 cases for donors who have donated more than six times (group III). Blood was collected for hematological parameters and for serum ferritin testing by immunological assay.Results: The results showed that the level of serum ferritin tended to decrease with the frequency of blood donation. The level of serum ferritin in blood donors who donated more than six times but did not take a full dose of iron supplements (43.2+25.0µg/L), statisticasignificant decreased when compared with the first donated group (75.4+40.2µg/L) (p=0.03). However, all hematological parameters were not significantly different among these 3 groups. Prevalence of iron depletion without anemia in group II and III were 6% and 3.4%, respectively. Only one donor in group III was iron deficiency anemia. Conclusion:This study shows that blood donation may lead to iron depletion and iron deficiency anemia, especially in more frequent donors. Iron supplements or dietary iron is required after a donation for iron level maintenance and iron deficiency prevention.
