Introduction: Brazil is an important consumer market for cocaine. However, the consequences of this consumption and the pattern of distribution of the estimates are still poorly studied in the Brazilian states. The Global Burden of Disease study -2019 (GBD-2019) has enabled us to describe and analyze indicators of mental disorders (MD) attributable by cocaine use in Brazil and its states, in 1990 and 2019. Methods: A descriptive study of the burden of cocaine use disorders, using prevalence, age-standardized mortality rate (ASMR), years of life lost (YLL) due to premature death, years lived with disabilities (YLD), and disability adjusted life years (DALY), which accounts for YLL+YLD. Results: Brazil ranks 8 th as DALYs due to cocaine use disorder in the world (42.83/100.000; 95% uncertainty intervals [95% UI]: 35.28 to 61.43). Significant increases have occurred in the age-standardized rate prevalence (ASRP), ASMR, DALY, YLD, and YLL, in Brazil and its states, between 1990 and 2019. The ASRP in 2019 was 2.7-fold higher for men (278.60/100.000; 95% UI: 208.20 to 374.39) in comparison to women (104.01/100.000; 95% UI: 76.70 to 143.02). There is a predominance of YLD in the composition of DALYs; however, the YLL had the biggest increases between 1990 and 2019. Conclusions: The high rate of DALYs and the increase in mortality rates show the need to scale up effective interventions to prevent and reduce the burden of disease attributable to cocaine use disorder, which is a preventable cause of death and disability.

Background To inform plans to achieve universal health coverage (UHC), we estimated utilisation and unit cost of outpatient visits and inpatient admissions, did a decomposition analysis of utilisation, and estimated additional services and funds needed to meet a UHC standard for utilisation. Methods We collated 1175 country-years of outpatient data on utilisation from 130 countries and 2068 country-years of inpatient data from 128 countries. We did meta-regression analyses of annual visits and admissions per capita by sex, age, location, and year with DisMod-MR, a Bayesian meta-regression tool. We decomposed changes in total number of services from 1990 to 2016. We used data from 795 National Health Accounts to estimate shares of outpatient and inpatient services in total health expenditure by location and year and estimated unit costs as expenditure divided by utilisation. We identified standards of utilisation per disability-adjusted life-year and estimated additional services and funds needed. Findings In 2016, the global age-standardised outpatient utilisation rate was 5•42 visits (95% uncertainty interval [UI] 4•88-5•99) per capita and the inpatient utilisation rate was 0•10 admissions (0•09-0•11) per capita. Globally, 39•35 billion (95% UI 35•38-43•58) visits and 0•71 billion (0•65-0•77) admissions were provided in 2016. Of the 58•65% increase in visits since 1990, population growth accounted for 42•95%, population ageing for 8•09%, and higher utilisation rates for 7•63%; results for the 67•96% increase in admissions were 44•33% from population growth, 9•99% from population ageing, and 13•55% from increases in utilisation rates. 2016 unit cost estimates

Verificar o índice de mortalidade em portadores do vírus da imunodeficiência humana (HIV) e/ou aids no estado do Pará entre os anos de 2007 a 2017. Trata-se de um estudo descritivo, transversal e retrospectivo, por meio da base de dados DATASUS (departamento de informática do sistema único de saúde) na faixa etária de 20 a 59 anos. Como resultados temos, a região metropolitana I apresentou maior prevalência de óbitos, seguida de Carajás. A idade onde se concentrou maior mortalidade foi em adultos de 30 a 39 anos, além de que o estado civil mais comum foram os declarados solteiros. Em relação ao índice de escolaridade, a maioria se concentrou entre 4 a 7 anos de estudo, e no que concerne a cor/raça, a parda foi a mais comum. A respeito da região Metropolitana I apesar de ter quantitativo menor de municípios foi a com maior mortalidade devido a questão populacional, a discrepância entre homens e mulheres se deve ao fato de que eles não procuram centros de saúde com assiduidade, já o tempo de estudo foi relativamente curto devido a problemas escolares/reprovações. Portanto, o HIV ainda é um importante marcador de mortalidade no estado do Pará, e os homens ainda são maioria nesses casos.
