External Validation of European System for Cardiac Operative Risk Evaluation II (EuroSCORE II) for Risk Prioritization in an Iranian Population

Abstract: IntroductionThe European System for Cardiac Operative Risk Evaluation II (EuroSCORE II) is a prediction model which maps 18 predictors to a 30-day post-operative risk of death concentrating on accurate stratification of candidate patients for cardiac surgery.ObjectiveThe objective of this study was to determine the performance of the EuroSCORE II risk-analysis predictions among patients who underwent heart surgeries in one area of Iran.MethodsA retrospective cohort study was conducted to collect the required v… Show more

“…These differences may be related to longer life expectancy and a lower incidence of rheumatic heart disease (more common among women in our country) than in European countries. To obtain results comparable to those of our study, we can consult the Asian series like that of Atashi et al, 12 that of Kar et al, 13 or that of Pillai et al 14 Since the population of EuroSCORE II is 9 years older than our population, there should be more comorbidities, but paradoxically, we found that diabetes on insulin was more common in our population while chronic lung disease was more common in the population of the original study.…”

“…Therefore, many models have been developed to predict these events ( 23 ). One of these applied models is the EuroSCORE II questionnaire that has a predictive value to determine early mortality after cardiac surgery ( 24 , 25 ). In the comparison of the two groups for EuroSCORE II, we observed no significant difference, which is compatible with Jamaati et al ( 23 ) and Atashi et al ( 25 ) studies.…”

On the other hand, there was a significant difference between the on-pump and off-pump groups in EuroSCORE ( 22 ).…”

“…The meta-analysis by Guida et al 3 , which included 22 studies, showed overall discrimination of 0.79, similar to that in our study. The most recent publications, not included yet in a systematic review, include some of the following: Iran 21 , China 22,23 , Greece 24 , Spain 25,26 , India 27 , US 28,29 , and the Netherlands 30 , with ROC areas ranging from 0.67 to 0.87.…”
