Histopathologic examination revealed a large subcorneal pustule filled with neutrophils. The epidermis showed minimal exocytosis of neutrophils forming small intraepidermal microabscesses. In the dermis a moderate perivascular infiltrate of lymphocytes and neutrophils without vasculitis was seen. Periodic acid-Schiff, Grocott, and Gram stains were negative. Direct immunofluorescence microscopy performed on the perilesional skin was negative.Based on clinicopathological findings, the diagnosis of subcorneal pustular dermatosis (SPD) was made. After ruling out glucose-6-phosphate dehydrogenase deficiency, treatment with dapsone 50 mg twice daily and topical corticosteroids was started. A complete response was achieved in 4 weeks, and the treatment was stopped.
