Pitted keratolysis

Abstract: Pitted keratolysis is a skin disorder that affects the stratum corneum of the plantar surface and is caused by Gram-positive bacteria. A 30-year-old male presented with small punched-out lesions on the plantar surface. A superficial shaving was carried out for scanning electron microscopy. Hypokeratosis was noted on the plantar skin and in the acrosyringium, where the normal elimination of corneocytes was not seen. At higher magnification (x 3,500) bacteria were easily found on the surface and the described tr… Show more

“…Pitted keratolysis is a bacterial skin infection of the foot, caused by excessive sweating and use of occlusive footwear. The infection is fairly common, especially when wet shoes, after ditch marches and other amphibious exercises, are worn for extended periods 18. Further, we speculate that higher acute workloads in the RNLMC (greater distances with heavier load) and/or timely access to military healthcare (exclusively marine training centre base) and/or group culture (eg, recruits are encouraged to visit the physician) may contribute to this finding.…”

“…Clinical features: The condition presents as small crateriform pits 1–3 mm in diameter, located on the pressure-bearing areas such as balls of toes and heel. [ 15 ] The most common sites of onset of PK are the pressure-bearing areas such as the ventral aspect of the toe, ball of the foot, and the heel. The next most common site is a friction area, the interface of the toes.…”
