Photodynamic therapy (PDT) is a noninvasive treatment that utilizes light treatment along with application of a photosensitizing agent. In dermatology, PDT is commonly used and approved for the treatment of oncological conditions such as actinic keratosis, Bowen disease and superficial basal cell carcinoma. In the last 2 decades however, PDT has also been used for the treatment of several nonneoplastic dermatological diseases. The present review summarizes published data on PDT application in skin appendage disorders. Our literature review shows that: (a) PDT may be a suitable treatment for acne, folliculitis decalvans, hidradenitis suppurativa, nail diseases, and sebaceous hyperplasia; (b) there is a lack of agreement on PDT features (type, concentrations and incubation period of used substances, number and frequency of PDT sessions, optimal parameters of light sources, and patient characteristics [e.g., failure to previous treatments, disease severity, body surface area involved, etc.] which should guide PDT use in these diseases); (c) further research is needed to establish international guidelines helping dermatologists to choose PDT for the right patient at the right time.

Toenail onychomycosis is a common infection, affecting up to 14% of the population. Moreover, poor penetration of topical drugs into the nail bed leads to frequent recurrences of the disease. The cure rate of toenail onychomycosis is estimated at 63-76%. An alternative treatment for onychomycosis is photodynamic therapy. Following agents are used in this method: porfimer sodium, rose bengal, methylene blue, 5-aminolevulinic acid and its derivative -methyl ester. The therapy is well tolerated Photodynamic therapy offers an opportunity to achieve a significant cure rate in patients who cannot receive oral therapy. Thanks to systematic improvements in photodynamic therapy, including better photosensitizer penetration into the nail bed and shorter exposure time, photodynamic therapy may be more efficient and cost-effective than other onychomycosis treatments. However, randomized clinical trials evaluating the efficacy of photodynamic therapy of onychomycosis are needed. STRESZCZENIEGrzybica paznokci stóp jest częstym zakażeniem, może dotyczyć do 14% populacji. Ponadto utrudnione przenikanie substancji leczniczych do łożyska paznokcia w leczeniu miejscowym powoduje częste nawroty schorzenia. Szacuje się, że odsetek wyleczeń grzybicy paznokci stóp wynosi 63-76%. Terapia fotodynamiczna stanowi alternatywną metodę leczenia grzybicy paznokci. W tym zabiegu wykorzystuje się następu-jące substancje: porfimer sodu, róż bengalski, błękit metylenowy, kwas 5-aminolewulinowy oraz jego pochodną -ester metylowy. Leczenie jest dobrze tolerowane przez pacjentów. Dzięki zastosowaniu terapii fotodynamicznej można uzyskać znaczny odsetek wyleczeń wśród pacjentów, którzy nie mogą przyjmować terapii doustnej. Dzięki systematycznemu ulepszaniu terapii fotodynamicznej związanemu z lepszą penetracją fotouczulacza do łożyska paznokcia czy też ze skróceniem czasu naświetlania może być ona bardziej wydajna i kosztować mniej w porównaniu z innymi metodami leczenia onychomykozy. Konieczne są jednak randomizowane badania kliniczne oceniające skuteczność leczenia grzybicy paznokci za pomocą terapii fotodynamicznej.

Przegląd Dermatologiczny 2016/3 246 STRESZCZENIE Grzybica paznokci jest częstą chorobą dotyczącą aparatu paznokciowego, a jej leczenie wg obecnie obowiązujących standardów nie zawsze przynosi zadowalające efekty. Celem pracy jest zwrócenie uwagi na nowe możliwości terapii z wykorzystaniem promieniowania laserowego, promieniowania ultrafioletowego typu C, a także terapii fotodynamicznej. Najwięcej danych piśmiennictwa dotyczy wykorzystania lasera neodymowo-yagowego, a także laserów diodowych lub ablacyjnego lasera dwutlenkowo-węglowego. Niektórzy autorzy, aby poprawić skuteczność leczenia, stosowali przed zabiegiem usuwanie mas grzybiczych lub dołączali miejscowe leczenie preparatami przeciwgrzybiczymi. Ponadto przedstawiono metodę fotodynamiczną wykorzystywaną w leczeniu stanów przednowotworowych i powierzchownych nowotworów skóry, która daje obiecujące wyniki w terapii grzybicy paznokci z zastosowaniem tych samych lub innych fotouczulaczy. Uzyskane przez autorów efekty leczenia i brak poważnych działań ubocznych zachęcają do stosowania tego typu terapii, ale żadna z metod do tej pory nie została porównana z leczeniem farmakologicznym wg aktualnych wytycznych. ABSTRACTOnychomycosis is a frequent nail disease. Standard treatment is not sufficient in some patients. The aim of the study is to provide an overview of laser, UVC and photodynamic therapy of onychomycosis. The use of Nd:YAG laser is described in many articles, and also diode lasers and ablative CO 2 are used. Some authors, in order to improve the efficiency of treatment, also administered topical antifungal therapy or removed a part of the nail. Also photodynamic therapy usually used in precancerous diseases or superficial skin tumors is also employed in onychomycosis with the same or new photosensitizers. The results and the lack of serious side effects encourage the use of these methods, although none has been compared to standard oral therapy.
