“…13 The subgroup analysis of the Drug-Eluting Stent in the Real World (DESIRE) registry, which assessed the late outcome after PCI with drug-eluting stents in diabetic and non-diabetic patients, confirmed these findings in Brazil. 16 In the present registry, drug-eluting stents were used in approximately one-third of the patients; this rate was not higher because the Brazilian Unified Health System does not yet provide these devices.…”

“…The DESIRE registry published its overall long-term results 11 stratified by previous diabetes diagnosis. 12 In the latter study, major adverse cardiac events occurred in 9.8% of diabetic patients and in 7% of non-diabetics, at a mean follow-up of 2.5 ± 1.4 years (P = 0.05). The observed differences in the incidence of events between the two registries are related to the different event definitions adopted, differences in clinical and angiographic profile of patients, and different follow-up times.…”

“…16 The SAPPHIRE registry shows rates of major adverse cardiac events consistent with other registries. 12,17 According to the present results, it is concluded that the use of DES is of fundamental importance for the percutaneous treatment of coronary artery disease in diabetic patients, especially for those who do not use insulin, considering the reduction of risk of serious cardiac events to levels similar to those of non-diabetic patients.…”

“…The presence of a higher number of diabetic patients in the second study period has an additional association with the increased supply of pharmacological stents in this period, providing a more effective approach with lower rates of restenosis in this population. 14 The implementation of chest pain protocols in hospitals participating in the registry, with primary PCI availability 24 hours a day, may explain the growing number of this type of PCI in the present registry. Regarding the care of AMI, the rate of use of glycoprotein IIb/IIIa inhibitors remained stable and low (6.4 % to 7.5 %) during the study period, which has been a trend in literature since the advent of clopidogrel, new antiaggregants, and the use of thrombus aspiration catheters 12 The more frequent use of aspiration catheters in the second period group coincides with the publication, in 2008, of the TAPAS trial results, which showed the superiority of thrombus aspiration technique to treat AMI with ST-segment elevation, reducing cardiac mortality combined with non fatal reinfarction within one year.…”
