Objective: Criteria for defi ning 'severe septal deviation' and to describe the clinical profi le of the same.Study: Retrospective study. Materials and methods: Hundred patients who were diagnosed with severe DNS and treated with extracorporeal septoplasty (ECSP) from September 2010 to December 2012, were retrospectively evaluated for this study. A review of their clinical charts formed the basis of this study. Results• In this study, majority of patients (96%) had nasal obstruction as their prime symptom followed by postnasal discharge in 60% cases, headache in 40% cases and anterior nasal discharge in 30% cases. External nasal deformity was reported by 22 patients. Snoring was seen in 24% of patients with same percentage complaining of altered sense of smell and throat discomfort. Epistaxis, sneezing and facial pain were seen in 14% patients. Epiphora was complained by only 8% of patients. • In this study, nasal endoscopy/anterior rhinoscopy was used to type the septal deformity. The commonest septal deviation was C-shaped cephalocaudal (48%), followed by S-shaped cephalocaudal (18%), C-shaped AP (16%), S-shaped AP (12%) and sharp septal deviation/angulation in 6% cases. • All but three patients (6%) had deviated nasal septum involving multiple Cottle's areas. These three patients had sharp septal angulation involving Cottle's area 2 only. • In this study, most common region involving DNS was area 1 + 2 + 3 (48%) followed by area 2 + 4 + 5 (28%) and 1 + 2 + 3 + 4 (18%). • Area 2 was invariably involved in 100% of cases. • NOSE (nasal obstruction symptom evaluation) scores. • Preoperatively, mean NOSE score was 67.60 ± 5.26 (65.34-72.86). • NSS (nasal symptoms score):-Preoperatively, mean NSS was -5.08 ± 0.38 (-5.46--4.70). Conclusion• A septal deviation is regarded as 'severe' if patient satisfi es all of the below-mentioned criteria: -Preoperatively, mean NOSE score should be 65.34 or more. IJHNS RESEARCH ARTICLE

Background: Nasal obstruction is a highly prevalent problem caused by nasal septal deviation, with or without inferior turbinate hypertrophy. Nasal septoplasty and inferior turbinoplasty can improve the nasal airways. This study was designed to assess the efficacy of septoplasty alone and septoplasty with inferior turbinoplasty in cases having nasal septal deviation with inferior turbinate hypertrophy.Methods: A total 70 cases clinically diagnosed with the deviated nasal septum and inferior turbinate hypertrophy were randomly divided into two groups. Group 1 cases were under septoplasty alone and group 2 cases were under septoplasty with inferior turbinoplasty. Symptom improvement scores was assessed by sino nasal outcome test-20 (SNOT-20). Results: The mean reduction symptom score of nasal obstruction was 1.73 in septoplasty with inferior turbinoplasty group, while in septoplasty group it was 1.06. The mean reduction symptom score in group 2 was statistically significant. All the cases of group 2 had adequate first pass diagnostic nasal endoscopy (DNE). While in group 1, majority cases had inadequate first pass DNE. Conclusions: First pass DNE findings were favourable in the cases under septoplasty with inferior turbinoplasty Septoplasty with inferior turbinoplasty is effective in the reduction of nasal obstruction, facial pain, sleep difficulty, decrease in attentiveness and irritability in cases having deviated nasal septum with inferior turbinate hypertrophy.

To compare the symptomatic improvement of nasal symptoms following septoplasty with partial inferior turbinectomy (groups A) versus septoplasty alone (groups B) and to assess the improvement of nasal symptoms in both surgical groups before and after surgery by NOSE scale. This Tertiary Hospital based study was carried out between August 2012 and April 2014. 60 cases with septal deviation and contralateral inferior turbinate hypertrophy. Nasal Obstruction Symptom Evaluation (NOSE) scale for evaluating nasal symptoms. Patients were alternatively divided into two surgical groups, group A. Septoplasty with partial inferior turbinectomy and group B septoplasty alone. Post-operative patient's symptoms evaluated by NOSE scale at 1, 3 and 6 months. Data analysed using tables, graph and percentage and test of significance like paired t test, Friedman test, Chi square test used. Post operative improvement following both group A septoplasty with partial inferior turbinectomy and group B in those undergoing septoplasty alone was highly significant (p < 0.001) at post-op 1, 3 and 6 months subjectively. When both groups were compared those undergoing partial inferior turbinectomy surgery with septoplasty had highly significant results (p < 0.001) for subjective assessment by NOSE scale. This study showed that hypertrophied turbinate need to be addressed in chronic cases of nasal obstruction with deviated nasal septum and contralateral turbinate hypertrophy. partial inferior turbinectomy should be done in addition to septoplasty, its highly effective modality for the treatment of nasal obstruction in patients with deviated nasal septum. NOSE score can be used as a subjective tool for symptomatic measurement of patients with nasal obstruction.
