Leakage tests reduce the frequency of biliary fistulas following hydatid liver cyst surgery

Abstract: BACKGROUND AND AIM:Biliary fistulas are the most common morbidity (8.2-26%) following hydatid liver surgery. The aim of our study was to reduce the incidence of postoperative biliary fistulas after the suturing of cystobiliary communications by applying a bile leakage test.PATIENTS AND METHODS:A total of 133 hydatid liver cysts from 93 patients were divided into two groups, according to whether the test was performed. Tests were performed on 56 cysts from 34 patients, and the remaining 77 cysts from 59 patient… Show more

“…This is just indicating a biliary communication in these cases which is certainly important to be identifi ed or/and confi rmed by a simple test. Kayaalp et al (2011) reported that, biliary fi stulas are the most common morbidity (up to 26 %) following hydatid liver surgery. Thus, raise the importance of rapid intra-operative verifi cation.…”

“…Stent insertion and nasobiliary tube drainage are usually suggested for high output and persistent fistulas. 27,[30][31][32][33] Stent insertion is better tolerated and avoids bile loss but requires further intervention for exchange and removal. Nasobiliary drainage allows cholangiography for control of the fistula and biliary tree but prolongs the hospital stay and causes patient discomfort.…”

“…If the patient has BBF as the presenting symptom, possibility of biliary involvement should be kept in mind and preventive measures should be taken to treat the involved bile ducts (13). The efficacy of intraoperative bile leak test during the surgery for hydatid liver disease have been shown previously (14). The present case report is the first study to emphasize the efficacy of intraoperative bile leak test in the treatment of complicated hydatid liver disease that resulted in BBF.…”
