Central venous catheters (CVCs) are widely used in various puncture and drainage operations in intensive care units (ICUs) in recent years. Compared to conventional operating devices, CVC was welcomed by clinicians because of the advantages of easy use, less damage to the body and convenient fixation process. We came across a patient with severe acute pancreatitis (SAP) who developed cardiac arrest due to thoracic cavity massive bleeding 24 h after thoracocentesis with CVC. Thoracotomy surgery was carried out immediately, which confirmed an intercostal artery injury. The patient was discharged from hospital without any neurological complications two months later. Here we report this case to remind all the emergency department and ICU physicians to pay more attention to the complication of thoracic cavity bleeding following thoracocentesis conducted by CVC.

La survenue d'un hématome fémoral après l'ablation d'un cathéter veineux central est une complication précoce et rare. Nous rapportons ici le cas d'un hématome fémoral survenant tardivement, 3 jours après l'ablation d'un cathéter fémoral, chez un patient de 63ans sous héparinothérapie à dose curative. Il s'agit d'un hématome volumineux mesurant 11.5 cm sur 8.2 cm sur 13 cm au niveau de la loge des adducteurs de la cuisse qui s'est compliqué d'un état de choc hémorragique. Une telle complication soulève la question d’éventuelles recommandations dans la surveillance d'une voie d'abord fémorale particulièrement chez les patients sous anticoagulation efficace.
Central venous catheter complications can be related to insertion, indwelling, or extraction. Most of the times, immediate complications are anticipated and managed; whereas, delayed complications can go unnoticed. In the case discussed here, migration and dislodgement of catheter tip resulted in delayed hydrothorax and sudden death of a 9-month-old female infant.
