“…In our patients, the guidewire was not found to pass directly into the venous system on radiological monitoring during the procedure. Hence, we considered that the nephrostomy tube passed into a vein after the fascial dilators had torn a large vein, as described by Mazzuchi et al (6).…”

“…None of our 3 patients received antithrombotic therapy or developed venous thrombosis. The anti-coagulation mechanisms can avoid the formation of atrial thrombi and consequent pulmonary embolism (4), and thrombotic phenomena are probably not observed due to the high blood flow and low venous pressure inside these veins (6). Thus, antithrombotic therapy is not necessary for these patients.…”

“…Mazzucchi and colleagues reported 2 cases, both of which were done at the end of PCNL procedures, and discovered few days later. 3 The authors withdrew the catheters, in the presence of a surgical team, with no bleeding. They explained that the mishap occurred due to an injury of a large venous trunk during the percutaneous silicon catheter insertion procedure, with the presence of an Amplatz sheath near the venous injury, which dislodged the catheter into the IVC.…”

“…In cases where misplacement of the tube is detected, depending on the postoperative time elapsed, relocation of the nephrostomy tube under fluoroscopy is strongly recommended, and the surgical team must stand ready to operate in case an open emergency procedure is required [ 9 ]. Our case illustrates that control on the hemorrhage could be achieved with the catheter being withdrawn in stages, and laparotomy could be avoided despite of the perforation into the major renal vein.…”
